Job summary

Farleigh Hospice is the leading specialist palliative care provider in mid Essex, providing specialist support to patients and their families in both the community via our multidisciplinary locality care teams, and our inpatient unit.

Main duties of the job

We are looking for a highly motivated Social Worker who is passionate about palliative care to join our locality-based community care teams. Are you a motivated, forward thinking, talented individual interested in delivering excellent specialist palliative care within a multidisciplinary team to patients and their families within mid Essex?

The successful candidate will join the extensive MDT to deliver holistic care to patients and provide essential support to carers and families. Both face to face and virtually. As a palliative care Social Worker, you will have the ability to see the whole person rather than their diagnosis. You will understand the connections of their lives and seek to act on the constraints and discriminations they experience. As you work alongside people in a strength-based way to problem solve and support.

You will be an empathetic individual with post qualifying experience of working within local authority Social Care and will have a particular interest in working with people and their families receiving palliative and end of life care. You will have the ability to work with adults, children and families to provide 1:1, couples, family and group support. You will support and advise patients and their carers to navigate both health and social care systems. You will have experience of dealing with issues of loss, safeguarding and managing complex psychosocial situations.

Job responsibilities

As a Social Worker, you will work within a multi-disciplinary team providing specialist advice and care, working with and supporting adults who are facing end of life, their families and those who they are close to. We are primarily a pre-bereavement service and as social workers we are interested in the psychological, emotional, social, cultural and spiritual dimensions of peoples lives because we believe that all these areas shape the way they think and feel and what matters to them.

Through holistic assessment, provide information, advice, psychosocial care and support to patients, their families and carers, liaising and referring on as necessary to appropriate internal and external health and social care professionals, including the Spiritual Care team.

Work in partnership with those receiving care, helping them to access relevant health and social care, information and support when they need it. Support people to access, identify, appraise and interpret complex information, to enable them to determine their own future.

Use advanced communication skills in delivering highly emotive information clearly, sensitively and unambiguously with staff,patients, families and professionals by face-to-face, telephone and/or electronic means.

Work collaboratively as part of the multi-disciplinary team (MDT) sharing the clinical caseload of patients and carers within the locality.

To work as part of the MDT but also work autonomously making decisions and recommendations involving highly complex factors, seeking advice from colleagues or/and clinical leadership team when required, in order to make sound decisions around palliative and end of life care.

To support management in training and mentoring a multidisciplinary workforce from novice to expert to work as one team.

To support and implement any streamlined locality processes and services by embedding quality improvement methodology including clear outcome measures to produce efficient and highly performing community services.

To support the Hospice in building strong relationships with external partners and stakeholders and promote the good reputation of the Hospice e.g. local trusts, CCG, health and social care partners, businesses and voluntary organisations.

Use social work skills to advocate and assist those in their care to access care, welfare benefits and grants, recognising and responding to the impact of diversity, disadvantage and discrimination.

Identify gaps and failings in services, systems and society in order to negotiate for appropriate equitable care
