Invicta Health CIC have an exciting opportunity for a Home Visiting Nurse (Band 5) to join our friendly team. 

Main duties of the job

Purpose of Job:

The Visiting nurse will undertake a range of nursing assessments and provide appropriate care/treatment in conjunction with GP’s and the visiting team independently as appropriate for the need, according to practice policy, protocols etc. Ideally the post holder will be experienced in wound care, palliative support, basic assessment skills including baseline observations and be prepared to work towards clinical consultation skills.

The Visiting nurse will strive to improve standards of care and participate in the development to meet the needs of our patient population.

Additionally, the successful candidate will be encouraged and supported to undertake any further training required to ensure maintaining of competencies and appropriate role development.

Job responsibilities

Main Responsibilities:

Clinical

Assist and perform routine tasks as follows (this list is not exhaustive)

  • Venepuncture
  • Routine Vaccination
  • Observations including Bp, Heart rate, Oxygen Saturations, Temperature and Respiratory rate
  • Urinalysis
  • New patient screening within care homes
  • Dressings
  • Minor injuries
  • Removal of sutures/clips
  • Wound care/ulcer care/doppler
  • Chaperoning
  • Undertake housekeeping duties including general tidiness
  • To complete Home visits assigned by senior clinicians and report back appropriately
  • To complete Post hospital Discharge visits in care homes
  • Palliative Care home visits
  • Care home visits
  • As detailed by QOF, assist in the delivery of effective implementation of management of chronic disease patients

Professional

  • Work in accordance with Nursing and Midwifery Council Code of Professional Conduct including NMC registration is valid.
  • Ensure you comply with requirements for revalidation and maintain the knowledge and skills you need for safe practice.
  • Continue professional development (CPD) to supplement existing knowledge to aid further autonomy within the role.
  • Disseminate updates in current evidence to all in practice, particularly new learning from study.
  • Recognise and work within the limits of your competence.
  • Work with GP colleagues and Nursing Team to deliver services in line with current evidence. E.G NICE, Research, Identifying resource implications to Operation Manager/Lead Nurse.
  • Contribute to the development of policies/protocols/guidelines within the practice and ensure compliance.
  • Work collaboratively within the practice and across professional boundaries, networking and building relationships to streamline care pathways using a patient centred approach that breaks down barriers and crosses boundaries between Acute Care, Community services and social care services to meet the needs of patients.

Administrative

  • Always ensure confidentiality.
  • Ensure accurate and legible notes of all consultations and treatments are recorded in patient notes.
  • Ensure the clinical computer system is kept up to date with accurate details recorded.
  • Attend and participate in practice meetings as and when required.
  • Ensure restocking and maintenance of clinical areas and consulting rooms.
  • Report any problems to the operations manager.

Organisational

  • Participate/initiate, conduct, analyses and disseminates audit and evaluation findings; implementing changes to improve patient care as required.
  • Participate in research as required in the practice.
  • Initiate/participate in evaluation of services delivered.
  • Assist in direct provision of service priorities in line with current national service frameworks, government documents and practice objectives.

Communication

  • As well as the Visiting team, there is a need to work closely with reception and administrative staff to ensure smooth running of the practice.
  • There is also the need to establish and maintain good liaison with other surgeries and agencies including secondary care.
