Main duties of the job

We have a unique opportunity to welcome two new colleagues to join our Supportive Care team and we would welcome your interest.

Our Supportive Care Coordinators will be responsible for Carer, Patient & Family support alongside our team of therapists, counsellors and practitioners currently delivering our Supportive Care Services across Somerset.

As a Supportive Care Coordinator, you will work collaboratively with wider hospice services and have experience working with multiple partner agencies in delivering high quality responsive care that is compassionate, respectful, and patient centred. Excellent communication and interpersonal skills are essential with this role, as is an ability to effectively plan and manage a busy workload. You will be able to demonstrate your experience of supervising, supporting, and motivating diverse groups of individuals with varying needs.

The successful candidate will have gained a Level 3 qualification in Adult Care or a Level 2 qualification in Counselling, or other relevant equivalent qualifications.

This post will include cross county working. Access to your own vehicle for work purposes is essential for this post.
