Job summary

Our MDTA’s work within the community team to support practitioners (registered nurse, Physiotherapist, Occupational therapist, social worker, Clinical Nurse Specialist), to provide person-centered support, care and treatment to patients, their families and carers across the community who have palliative and end of life care needs. 

Main duties of the job

The ideal candidate for this role will be able to demonstrate experience as a health care assistant or physiotherapy/occupational therapy assistant, within a clinical environment and be confident in contributing to the multi- disciplinary plan of care for patients, families, and carers for whom the post holder has delegated responsibility.

As part of the MDTA team you will assess non-complex symptoms which may include pain, shortness of breath, mobility, pressure risk, continence and psychological factors. You will be required to analyse this information and carry out non-complex interventions within own scope of practice and refer on to specialists as needed.

The role will require travelling across Somerset including into rural communities and working unsocial hours, evenings, weekends, and bank holidays.

  • Demonstrable experience as a Health Care Assistant in a clinical environment, or an assistant working in physiotherapy or occupational therapy or equivalent Demonstrable experience of working and contributing in a multi-professional team. Experience of assessing patients and analysing this information in order to formulate a treatment plan and take appropriate action.

Desirable

  • Experience of teaching / presenting to groups. Previous experience in End of Life care Experience in rehabilitation Experience or understanding of coaching. Experience of working as an independent practitioner.

Qualifications

  • Access to own vehicle in order to be able to travel to various locations across Somerset. Ability to work over a 7 day shift pattern, including evenings, weekends bank holidays. Physical capabilities to be able to carry out the role required, this will include an element of moving and handling of equipment and the ability to walk reasonable distances and to stay standing for prolonged periods of time.
