Job Role
Clinical
To provide clinical navigation, and advice to appropriate clinicians
To reassure the rapid and effective transmission of patient data between
all relevant parties
To deal effectively with safeguarding issues if they arise in line with
policy.
Promote evidence-based practice
Establish and maintain strong collaborative arrangements with other
health care professionals
Participate in admission avoidance activities
Prescribe medication patients using EPS
Provide advice to professionals about
alternatives to hospital care, including primary and intermediate health and
social care (including voluntary) services
Provide brokerage role discussing
capacity with services and liaising with services to free up capacity dependent
on wider pressures in the system as part of escalation plans so as to avoid unwarranted admissions.
Make safe, autonomous triage decisions
without reliance on digital or algorithm based tools.
Managerial
To assist the clinical lead nurse to train, mentor and coach the
clinical team where relevant.
To participate in the appraisal process for themselves, and facilitate
for their team
To diffuse challenging behaviour, ensuring that the situation is managed
in a sensitive way.
To inspire others and encourage them to seek advice and solutions to
problems.
To challenge others to take an active part in developing knowledge,
ideas and work practice
To challenge tradition and take risks accepting joint responsibility for
any arising problems and tensions and using these to inform future practice.
Manage the teams rota
Communications
To communicate with a wide range of people to explore complex health
issues and to make treatment pathway decisions
To effectively communicate complex and sensitive information.
To agree the arrangements for communication with the
patient/family/carer, and to document these in accordance with policy and the patients right to
confidentiality
To ensure data collection systems are effective and report an problems
to the IT team.
Partnership workingand service development
To work in an integrated and seamless way with other colleagues and
disciplines within the Primary Urgent Care Services.
To adhere to policies and procedures relating to own workplace to
contribute to service development
To implement and propose changes to policies/practices in own area
To input into the effectiveness of any changes and how these have
improved services
To ensure collaborative working relationships with acute and primary
care agencies are effective
Training and Development
To facilitate and participate in an open, reflective and learning
environment.
To understand the role and scope, identify own development needs and
take responsibility for their continuing professional development and
performance whilst maintaining a personal development plan
To make effective use of appropriate learning opportunities for
themselves and others and apply learning to practice
To undertake annual mandatory training updates and other relevant
courses in line with local policies
To act as a role model and train and support professional development of
other staff
Engage in and comply with the requirements for nurse revalidation
Professional
Exercise individual responsibilities and accountability for own
practice, in accordance with clinical governance and professional guidance from
NMC.
Keep up to date with current initiatives which enhance the role

Participate actively in audit programmes of practice and service
improvement
To work together with the complaints team to address user complaints.
Organisational
Responsibilities
To ensure efficient and effective use of material resources/supplies
within the centre
To identify any problems with resource use/availability and make
recommendations for corrective action
To maintain accurate records of resource use
