Practitioner (ACP)
This is an
exciting time for PC24 as we are transforming and implementing new models of
care across our services and increasing our input into providing support to
intermediate care facilities.
This clinical role will require
the postholder to have extensive experience in assessing the needs of older
people, including being able to work independently within the nursing and
residential care home environment.
Main duties of the job
The postholder will be required to
undertake allocated review of patients via ward rounds using advanced
communication skills to discuss and complete proactive individualised clinical
management plans, with the aim being to reduce unnecessary hospital re-admissions.
In addition, they must have excellent
interpersonal skills to promote positive
working relationships with care home staff, community
nursing teams, mental health services, palliative care teams and social
services, to enable residents to meet their health goals
and be cared for in their preferred place of care.

Key responsibilities
Demonstrate high level of competence/advanced skills in clinical examination and diagnosis. Undertake ward rounds and individual review of patients as required
Interpret pathology/radiology results/reports and action where appropriate
Have excellent interpersonal and communication skills to provide enhanced discussions with resident and families regarding end of life wishes/planning
Embed core values and expectations of care, compassions, courage, communication, commitment, and competence into everyday practice
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national/local protocols and within scope of practice.
About you
You will be an experienced ACP who is able to deliver quality individualised care to our patients with a high level of autonomy underpinned by the four pillars of Clinical practice, leadership and management, education and research. You will be expected to utilise your skills in advanced clinical assessment and diagnostics reasoning to make complex decisions to determine and initiate changes to treatment. Flexibility, compassion and a can-do approach is essential for this position. The successful candidate will need to fulfil all the role requirements to be shortlisted for interview.
You will have a masters in advanced clinical and able to undertake comprehensive assessments, diagnose plan and prescribe medication. You will have experience of working in the acute sector, frailty services or in the older person environment and undertake comprehensive face to face clinical examination via ward rounds and individual review. You will practice autonomously whilst demonstrating a person-centered approach to meet PC24 patient health care needs.
