Job responsibilities
Main Role & Responsibilities:
Maintain an up to date
understanding of all PCN contracts and service specifications and ensure all
relevant legal and contractual requirements are met and best practice is followed.
Facilitate outcome
focussed innovation and the adoption of transformation improvement projects,
including integrated working at neighbourhood and/or place level to improve
patient access and health outcomes for the population.
Build relationships
and facilitate collaboration between practices, stakeholders and the wider
system to support the delivery of care to patients.
Lead an improvement
approach to change, including building capability for quality improvement
within the PCN, and neighbourhood.
Be a champion for best
practice, using data and analysis to inform and enable innovation and
transformation within the PCN.
Review of the PCNs
digital maturity, and lead improvement including optimisation of new
technologies to enhance patient access and improve efficiency within the PCN.
Specific Responsibilities:
Evidence Based Transformation and Population Health
Improvement
Work within the PCN
Executive team and with key stakeholders to identify where improvements can be
made via outcome focussed transformation projects.
Use Business
Intelligence, data and local information to identify opportunities in improving
population health and reducing health inequalities, within the neighbourhood
and locality plans, collaborating on relevant projects with partners.
Adopt a robust
programme management approach to deliver transformation, ensuring that any
risks, conflicts and interdependencies are managed effectively.
Position the PCN as
integral to successful working within the neighbourhood by understanding the
patient population, the assets and partners, and building strong relationships
with other key leads, services and stakeholders.
PCN Service
Development and Achievement
Maintain knowledge and
understanding of core contracts, Direct Enhanced Services (DES), Impact and
Investment Fund (IIF), Primary Care Standards (PCS) and Quality and Outcomes
Framework (QOF) indicators; ensuring staff understand their role in achievement
of these; and support provision of training and feedback to ensure targets are
met and, where possible, exceeded.
Agree appropriate
systems for delivery of services and ensure implementation through robust
monitoring and quality reviews.
Support workforce planning for
the PCN by
considering the future workforce needs in relation to delivering the PCN
vision, priorities, specifications, and growth.
Ensure transformation projects include meaningful and valued engagement
and involvement with users of its services, and that this is fully embedded in
the development of services within the PCN and neighbourhood.
Quality Improvement and Data Analysis
Use recognised
frameworks to drive quality improvement in a structured way, by leading and
advocating quality improvement initiatives.
Interrogate and
analyse, often highly complex information to support the delivery of projects
and write and present regular high-level performance reports for the PCN Board
demonstrating progress, identifying issues, and making evidence-based
recommendations.
Use data and
information to understand demand, capacity, activity, and to achieve set
targets to deliver projects and programmes in the PCN.
Embedding Operational
Efficiency including Digital Innovation
Identify potential
areas for collaboration across the PCN practices to gain efficiencies, improve
services and deliver high quality patient care.
Appraise digital
options and technologies for general practice as they develop/emerge and
encourage practices and PCN in utilising their full functionality to enable
improvement.
Engage with Digital
Action Learning Sets and work with NHS GMIC (Wigan) Digital Team on embedding
new models of care and in supporting practices and the PCN.
Other Responsibilities
Work closely with the
PCN Manager to ensure that strategy and operational delivery are aligned and
reflect the vision of the PCN.
Attend appropriate PCN meetings as required and
wider meetings, including but not limited to Wigan locality and partnership,
SWAN neighbourhood and Greater Manchester.
Horizon scan for opportunities, barriers, threats
and changes, assessing these and bringing them to the attention of the
Executive team for consideration.
Undertake any training required to maintain
competency including mandatory training.
