Job summary

We have an exciting opportunity for an experienced Social Worker to join us, leading our small Social Care team and being the Designated Safeguarding Lead for the Hospice. The successful candidate will be responsible for overseeing the Social Care team in providing expert knowledge to patients and carers across Hospice services, including social care assessment and discharge planning.

Main duties of the job

Working in collaboration with the wider MDT, you will manage a complex workload to deliver on key objectives ensuring that key quality standards and compliance targets are met.

You will have a relevant degree and demonstrable experience of both managing complex caseloads and leading a team to deliver a busy workload.

With a sound knowledge and understanding of relevant legislation and Safeguarding responsibilities, you will also have excellent interpersonal and communication skills combined with strong organisational and administrative skills. Experience of working with people affected by life limiting conditions, in palliative care or a healthcare setting preferable.

The hours will ideally be worked over 3 or 4 days across the week. This appointment is subject to an Enhanced Disclosure and Barring Service check, including Barred lists. Key Accountabilities:

1. Maintain the highest levels of expertise in specialist Social Work practice, providing support to patients, families, clinical leads and members of the Hospice MDT.

2. Manage own caseload, engage with patients and their carers and aid facilitation of potential social care options/solutions in conjunction with the wider MDT.

3. Oversight and management of the assessment of social care needs and future care planning / discharge planning process for IPU patients, working in partnership with colleagues from community social services to facilitate complex discharge management and prevent avoidable readmissions.

4. Line management of the Social Care team of registered Social Workers and a Social Care Assistant.

5. Development and maintenance of strong working relationships with MK Social Care services, the hospital-based palliative care team, and other relevant teams, including representation of the Hospice at internal and external meetings as required, maximising the profile of Willen Hospice and bringing best practice back to the service.

6. Keep up to date with registration requirements, legislation including the Health and Care Act and legislation rights of patients and carers and advise the MDT as appropriate.

7. Operational Lead for Safeguarding and Deprivation of Liberty in line with Willen Safeguarding Strategy.

8. Development and implementation of Willens Carers Strategy, contributing to oversight of programme delivery within the Living Well Centre. Working collaboratively across the hospice to raise the profile of needs of carers.

9. Contribute to and participate in audit and clinical governance activities and attend Clinical Effectiveness meetings to ensure continuous service and care quality improvement.

10. Provision of social work activity data as required, contributing to KPIs and other ad-hoc statistical information requested by Willens senior management and leadership teams, the Integrated Care Board and governance mechanisms.

11. Working with senior management team on the strategic development of Social Work responding to local, network and national developments particularly in light of the growing population of Milton Keynes and underserved groups.
