This is a great opportunity for an enthusiastic doctor to join our
established medical team, who provide the highest quality of Specialist
Palliative Medicine across Milton Keynes and the surrounding area. You would join our current team which
includes one Consultant, a GP with specialist interest, a Hospice Doctor, two
GP VTS trainees and a FY2 doctor - in what is a developing and expanding
service.
Main duties of the job
The role would primarily be to
support our community palliative care team Willen at Home, providing
palliative care support to adults with life-limiting illness across Milton
Keynes. The role will require you to work as part of the Hospices experienced
multi-professional team, and you would have the opportunity to develop
experience across all our services.
This post
is fixed term for 12 months, but there is potential to make this permanent. It
would be ideally suited for a GP who wishes to develop specialist interest and
skills in palliative care. There are up
to four days per week available, but this could be shared between 2 or more
doctors.
This post includes weekend cover,
working 1:4 weekends. Weekend work involves a ward round 9am-3pm on Saturday.
Ideally, this would be on-site support to the trainee doctor and Willen at Home
Team. There is no on-call commitment to this post.
Although the post would be particularly
suitable for colleagues with a background in General Practice, applicants with
experience in Hospital Medicine (having completed Foundation Training) would be
considered. The successful candidate will have full GMC registration with licence
to practice, experience to demonstrate foundation competencies or above, and an
understanding of how the NHS works.

Willen Hospice is situated in a beautiful lakeside
area close to the centre of Milton Keynes. The Hospice is the leading provider of specialist
palliative care within the area. In addition to our newly refurbished 15 bed
In-Patient unit and Community Palliative Care Team (Willen at Home and the Hub),
we have an expanding Therapeutic & Wellbeing service offering Psychological
Wellbeing, Social Care, Physiotherapy, Complementary Therapy, Spiritual Care
and a new Living Well Centre.The hospice medical team provides
specialist medical care and advice to all these areas and has a close working
relationship with Milton Keynes University Hospital.

KEY DUTIES AND RESPONSIBILITIES
The post holder will work as part of the medical team and will undertake a range of clinical duties dependent on the needs of the service. The post holder will be supported in their role by the Consultant/Clinical Lead and the existing medical team
The duties will be varied and will primarily be to provide clinical support to the community palliative care team.
The post holder will be expected to:
- Provide medical advice and support to the community teams of Willen at Home nurses in the form of joint home visits, out patient consultations and case discussions.
- Assess, investigate and plan the medical management of new patients or patients already under the care of hospice services with evolving problem(s).
- Liaise closely with the hospice community nursing teams and external community health/social care teams to optimise patient management and to ensure that medical treatment plans are communicated appropriately.
- Communicate efficiently and effectively with community health and social care colleagues, and hospital teams.
- Communicate with other agencies as required.
- Provide medical advice by telephone at the request of external health professionals (in keeping with their experience).
- Communicate effectively and appropriately with patients, their relatives and/or those important to them.
- Show dignity and respect to patients, their families and work colleagues at all times.
- Maintain confidentiality at all times.
- Maintain good medical records.
- Undertake 1:4 weekends as part of their regular duties, working between 9am to 3pm on Saturday OR Sunday. Weekend duties are primarily to undertake a review of patients on the Inpatient Unit, admit new patients when needed, and provide advice and support to Willen at Home Team if required.
In addition, the post holder will be required to provide cross-cover to the inpatient unit and Wellbeing Centre. They will also contribute to the work of the wider organisation. This may include:
- Assess, investigate, plan, implement and evaluate inpatient care.
- Maintain accurate inpatient drug charts (using the electronic prescribing system).
- Liaise with Community team (Willen at Home team) at admission, discharge, and as indicated during inpatient stay.
- Communicate effectively with other health/social care providers (for example, GPs, hospital teams, Adult Social Services) about inpatients including the provision of timely discharge letters and telephone discussion where appropriate.
- Provide medical support to the Wellbeing Centre, including the assessment of new medical problems, at the request of the Wellbeing team.
- Attend and contribute to the MDT meetings.
- Participate in the education programme and provide teaching where appropriate.
- This post is for 6-8 Programmed Activities [PAs] per week, plus 1:4 Saturdays.
