Senior/Lead PCN Clinical Pharmacist

Job summary
To participate in the provision of a Clinical Pharmacy Service to our member GP Practices defined within the Primary Care Network geographical location.
Applicants should have completed 18-month CPPE Pathway course and be currently undertaking or completed the independent prescriber course.
Main duties of the job
The role will involve working towards various quality improvement projects and to achieve PCN network DES.
The role will have a patient facing element either face to face or telephone - including chronic disease management, repeat prescription management and structured medication reviews. This will also include the management and review of patients with polypharmacy.
The post holder will work within their clinical competencies to provide specialist pharmaceutical support to all our patients and will be working with members of the wider healthcare team to improve care and support safe and effective prescribing.
The post holder will need to be proactively involved in quality improvement and audit as well as managing aspects of the Quality and Outcomes Framework.
The post holder will act as the first point of call for queries from pharmacists within the PCN and provide leadership skills to coach junior members of the team.
