Job summary
RHA has an exciting opportunity for an experienced and
driven individual to join our organisation to support our local primary
care networks and take on
the role of Digital Transformation Manager.
The post holder will be a key
member of the team and the role will include supporting our member practices to
deliver the strategic and operational aims of the PCN.
The post-holder will be expected to
develop and establish strong collaborative relationships with practices and all
stakeholders. Internally these include practice members of the PCN and RHA and,
externally, the CCG, and most importantly, the
PCN patients.
As the role of Primary Care
Networks, within the NHS, develops it is envisaged that the demands and
requirements of the post will increase over time.
The job description and person specification are an
outline of the tasks, responsibilities and outcomes required of the role. The
job holder will carry out any other duties as may reasonably be required by the
Clinical Director. The following areas of responsibility are an indication of the role but
could be subject to change as the
impact of primary care commissioning emerges.
Main duties of the job
The core objective
of the PCN Digital and Transformation Manager role is to support the PCN and
its practices to deliver ongoing improvement to services.
To
play a pivotal role in the delivery
of high quality primary health care
within the Primary Care Network (PCN).
To develop
strategy related to transformation and improvement.
Support the PCN Clinical Director
and the member practices to ensure the smooth
running of the PCN.
To support the production of
performance and quality reports within the PCN
and other Network initiatives, liaising with other stakeholders as required.
To be able to
interpret large quantities of data and disseminate this to relevant parties.
About us
Rochdale Health Alliance was established in 2016 by GP practices from across the Rochdale Borough to streamline the way in which services are delivered. Each of the 3 areas of the borough elected GPs to represent their locality and become directors on the board. The board is supported by a management team who ensure the day to day functioning of the organisation.
GP practices across the Rochdale borough, who have historically worked independently, have came together to work in a federated model to improve healthcare across the Heywood, Middleton and Rochdale (HMR) neighbourhoods.

Key Responsibilities and Duties:
First point of contact for PCN
Practice Managers regarding delivery of PCN projects/schemes.
Lead on IT
projects.
Create
dashboards for relevant PCN data to be delivered in stakeholder meetings.
Support the induction of new PCN staff.
Management and administrative support
of monthly meetings.
Work alongside practices to
understand current challenges to effective service delivery and identify
solutions, including existing best practice within the PCN.
Recording and management of holiday and sickness of PCN staff.
The post holder will also
develop relationships with local delivery partners (such as IT service
providers and other NHS organisations) to support effective delivery of
transformation.
Work alongside practices to
understand current challenges to effective service delivery and identify
solutions.
Build relationships across the PCN to develop
consensus for improvements and to ensure business and clinical staff are fully
engaged.
Support staff to identify and participate in
relevant training and/or professional development opportunities.
Build and maintain good working relationships with a
broad range of internal and external stakeholders.
The post holder will be responsible for the
development, management and maintenance of systems and processes to ensure the
PCN operates at the highest level of performance and accountability.
The PCN Digital and Transformation Manager will form
an integral part of the PCN leadership working alongside the Clinical Director
and PCN member practice colleagues.
The post holder will act as the main point of
contact of expertise, coordinate the successful achievement of all PCN contractual
requirements, support the translation of national local policies into meaningful
actions, and support collaborative working to achieve improved outcomes for the
local population.
Analyse service specification, performance
indicators and data collection requirements to inform design for new PCN services.
Collate feedback/analysis data on behalf of the PCN
to report as required.
Help develop joint working and relationships across
networks and its stakeholders.
