Job summary

Our specialist team provides care for people with complex symptom management arising from life limiting illnesses. The care is provided within the home setting and also includes phone support for carers of the individual.

Our Community Clinical Nurse Specialists work autonomously, managing a caseload of patients with specialist palliative care and end of life care needs in the community. This role involves providing palliative nursing interventions working closely with medical and other colleagues, both internally and externally, and leading the entire Community Team ensuring a coordinated and effective service in the hospice and community setting.

Main duties of the job

We are looking for a self motivated, kind, caring and experienced registered nurse who is both excellent at communicating and has good assessment/prioritisation as well as leadership skills. The nature of the role means it is important to understand the needs of those receiving palliative or end of life care and motivate a team of specialist nurses.

The successful candidate will need to be able to assess the referrals appropriateness and urgency and be able to identify nursing best practice developments, that will support the clinical team. The role is ideally suited to someone with both community experience and clinical palliative care knowledge, who is able to work autonomously and demonstrate advanced nursing practice, promoting skilled, professional and compassionate nursing care within the community.

Own transport will be essential.

St Peter & St James Hospice is set in 36 acres of beautiful countryside and employs approximately150 employees, has support from around 400 volunteers, and its 11 charity shops across Sussex including the towns of Lewes, Haywards Heath, Uckfield, Lindfield and Burgess Hill and Distribution Centre in Hurstpierpoint.

Our friendly, professional and nurturing team of employees and volunteers work side-by-side to provide specialist hospice care for adults towards and at the end of life. We support patients and their carers in their journey both in the comfort of their own homes as well as at the hospice in North Chailey.

Job Description Position

ROLE PURPOSE - To coordinate the operational management of the community specialist palliative care team, who support patients and those most important to them, in their preferred place of care and death.

To provide managerial and leadership guidance and support to the team of CNSs, Development CNS, Registered Nurses, and Student Nurse Associates as required.

To support the Care Direct to deliver the clinical, strategic, and operational objectives of the organisation.

To act as a resource and support to health and social care providers involved in the delivery of palliative care through the sharing of advice, information, and education.

PRIMARY DUTIES AND KEY RESPONSIBILITIES

Provide advanced level palliative nursing interventions and ensure a coordinated and effective service in the hospice and community setting.

Advise and support Health Care Professionals with advanced symptom control, ethical issues, and highly complex nursing interventions.

Provide day to day operational coordination and management of workload allocation to meet fluctuating demands, liaising with other teams to ensure equity of resources, prioritisation, and effective time management.

Lead regular and effective multidisciplinary caseload management meetings, ensuring attention to detail and appropriate management plans are in place.

Participate in the Senior Nurse Manager On call service (on a rotational basis).

Develop and support the team through the continuous performance review programme and regular 1 to 1s, acting as a role model and demonstrating strong leadership and team player skills and appropriate delegation skills, to empower them to develop both professionally and personally.

Participate in promoting clinical effectiveness, quality assurance and practice development activities, which will include audit, benchmarking, service review and clinical supervision.

Ensure outcome measures demonstrate the effectiveness of hospice care, which is in line with the CQC single assessment framework.

Identify and initiate risk assessment processes to determine risks to health and wellbeing of patients with complex care needs, taking action to minimise risks.

Work proficiently and effectively within the relevant legal, ethical, and professional frameworks applicable to patients palliative care practice in the community at all times, including the NMC Code of Practice and Care Quality Commission

Be an active member of the multi-professional team, working closely and collaborating with colleagues and wider Hospice team to ensure patients needs are met to a consistently high standard. To attend planning, review and debrief meetings as appropriate.
