A rare opportunity has arisen within the Social Work Team to work across the LD/ASD Division at St Andrews Healthcare.

The ASD/LD divisionprovides medium secure, low secure and rehabilitation settings for men and women with a Learning disability and/or Autism and complex mental health needs. Main duties of the job

The role offers the opportunity to support patients with complex needs, through to successful discharge. Regular supervision is provided and opportunities for development both in social work and other related practice issues are provided and encouraged.You will be registered with theSocial Work Englandand there will be opportunities for training to ensure re-registration. You should be in receipt of the Degree in Social Work or equivalent and have experience with Mental Health Legislation including the Care Programme Approach. A good working knowledge of the Mental Health Act, the Mental Capacity Act and the SafeGuarding processes under the Care Act 2014 will be required.

Key Responsibilities: To manage a workload as agreed by the Lead Social Worker and MDT. To undertake work as agreed in relation to The Mental Health & Mental Capacity Act. To prepare and present reports for Mental Health Review Tribunals and Managers Hearings. To prepare and present Social History reports and reports for Clinical Reviews. To attend relevant multi-disciplinary meetings and liaison with community social workers, other agencies and professionals. Engage in 1-1 and group work with service users and maintain up-to-date records in clinical notes of all service user contacts. Liaise with service users families / carers and other statutory / voluntary agencies. Undertake child visit and home visit risk assessments making recommendations to the MDT. Lead on planning for service users discharge and aftercare arrangements including aspects of safeguarding and multi-agency public protection arrangements. Identify, visit and assess resources that may be considered appropriate for service users moving into the community and or other residential settings. Adhere to MAPPA processes, making referrals and attendance at MAPPA meetings where appropriate. Undertake S42 enquiries as directed by the Local Authority. Lead on the day to day co-ordination of safeguarding. Attend and take an active part in Social Work team meetings. Regularly attend prearranged supervisions with Lead / Senior Social Worker with prepared agenda items.
