We have a new and exciting opportunityto work with ourteam as a Team Leader working in our Integrated Care Team to lead and develop our Community provision. Our teamare committed to delivering the highest possible standards of respite, palliative and end of life care to babies, children and young people. Working at Derian House will involve working with a small team of dedicated professionals at all levels and also provides the opportunity to make a real difference to the babies, children and young people we support and their families.
Main duties of the job
Main Purpose of Post:
To lead in
the development and operational management of the Community offering at Derian
House Childrens Hospice by providing support and guidance to all members of
the team.
To practice
and promote advanced specialist palliative care, encompassing a supportive and
holistic approach to the child/young person and their family with a
life-limiting condition in their home environment.
The post
holder will maximise the use of the multi-skilled team within the community
setting and the multi-skilled team within the Hospice to maintain continuity of
care for the child/young person and their family and in line with Derian House
Values.

Main Duties and Responsibilities:
Managerial:
- To support the Head of Integrated Care in the operational management of the service by providing leadership and management to the Community offering.
- To support the Director of Clinical Services in the leadership of care delivery; ensuring that the service meets and exceeds the requirements of the Care Quality Commission and any subsequent regulatory body.
- To act as a clinical supervisor and lead the Community offering ensuring all organizational clinical policies, procedures and guidelines, are implemented and reviewed in line with current legislation and professional guidance, as appropriate.
- Ensure the staff duty rota is implemented effectively and efficiently to provide the staffing levels required to meet the needs of the day to day operations within the Community offering.
- Assist the Head of Integrated Care to ensure that all Community staff are appropriately skilled to meet the needs of the CYP.
- Act as a role model in the supervision of the Community staff in setting high standards of care, conduct and professionalism.
- To regularly meet all members of their team both individually and as a team to provide, support, coaching, monitor performance and delegate specific tasks and roles.
- To ensure the economical and appropriate use of all resources within the organisation.
- To be responsible for the safe use, storage and maintenance of equipment/technical apparatus.
- To maintain appropriate client records ensuring they are stored securely and confidentially.
- To attend and participate in hospice meetings, as appropriate.
- To participate in the recruitment and selection of appropriate members of staff.
- To conduct research in specific areas of care/practice as and when required.
- To act as a positive role model by using evidence/research based care and practice.
- To participate in the delivery of in-service training programmes and teaching sessions.
- To participate in the orientation of new staff, students and volunteers. Acting as mentor as and when required.
- To identify ways of continued quality improvement, using reflective practice through Clinical Supervision and appraisals.
- To act as an appraiser for junior members of staff.

- To be proactive in identifying ways of improving practice. Contributing positively to the development and audit of standards and, when appropriate, taking responsibility for identified project work.
- To attend regular meetings with the multi-disciplinary teams to discuss individual children, respecting and inviting the sharing of skills, knowledge and expertise.

- Paediatric competency.
- Evidence of complying with NMC PREP requirements.
- Evidence of continued professional development.

- Palliative care qualification.
- Community nursing qualification.
- Willingness to study to degree level.
