Main Duties and
Responsibilities:
Professional
Act at all times in accordance with
relevant legal, ethical and professional legislation and frameworks (i.e.
Children Act).
Contribute to the development of a
culture that is committed to the continuous improvement of quality care and
patient experience.
To take responsibility for maintaining
own personal and professional development, meeting with the line manager for
regular supervision, annual appraisals and identifying/agreeing personal
targets and education needs.
Ensure that personal actions and
conduct comply with hospice policies and procedures.
To maintain confidentiality in all
aspects of the role, notably information regarding the babies, children, young
people (BCYP) and their families, staff and the organisation itself. Disclosure to any unauthorised person is
considered a disciplinary offence.
Foster a culture of respect and
consideration between all team members, respecting and nurturing the strengths,
skills and knowledge of others as well as sharing own knowledge and skills.
To attend regular meetings with the
multi-disciplinary team to discuss individual babies, children, and young
people respecting and inviting the sharing of skills and expertise.
Maintain relevant, accurate and
contemporaneous records and reports, respecting and maintaining confidentiality
at all times in accordance with NMC Guidelines, Derian House Childrens
Hospices policies & procedures and the Data Protection Act (1998).
To participate in supervision/support
regularly and consistently.
To be flexible to work within areas or
departments to meet the demands of changing service needs
Recognise and work within own scope of
practice.
Clinical
Work as part of the Integrated Care
Team delivering care that focuses on the direct needs of the individual and
their family.
Support people to improve and maintain
their mental, physical, behavioural health and wellbeing, promoting health and
preventing ill health.
Develop an understanding of all
elements of the nursing process and be able to assist the registered nurse in
the ongoing assessment, planning, management and evaluation of care.
To recognise changes in a BCYPs
condition which require the intervention of others and understand when to refer
to the supervising registered nurse and other senior colleagues for
reassessment of patient care.
To perform procedures as detailed
within the NMC Standards for proficiency for Nurse Associate (2018) (Annexe B)
to meet the nursing needs of the children/young people in their care.
To work within the NMC code of
professional practice, and within policies, procedures and guidelines of Derian
House Childrens Hospice (DHCH)
To have a willingness to undertake additional
skills and training dependent on the organisations requirements.
To ensure that high standards of
nursing care are given and maintained. To act and raise concerns when standards
are not being maintained following DHCH policy and procedures.
To act as an advocate for the BCYP and
the family, including promoting and protecting their interests, privacy and
rights, equality and inclusion.
To be involved in DHCH initiatives to
improve patient safety and care.
Recognise issues relating to
safeguarding vulnerable BCYP and report any problems or raise concerns to the
appropriate registered care professionals
Assist in the assessment of and
contribute to the management of risk across several areas within the
environment where care is being administered
Assist in the delivery of complex care
as prescribed by the registered nurse
Develop understanding of caring for
individuals with particular conditions for example; complex health needs,
learning disabilities, palliative and end of life care needs
Develop skills in relation to
coaching/teaching individuals/carers/other staff
Assist with the implementation and
monitoring of clinical standards and outcomes
Develop a working knowledge of other
providers resources and referral systems to ensure individuals needs are met,
within parameters of practice
Leadership and Management
To support the Registered Nurses in
the day to day operational management of the service by providing leadership
and supervision for Clinical Support Workers and Students.
Be resilient to manage change within
the organisation
To recognise, prevent where possible
and manage areas of conflict reporting to senior staff if unable to resolve.
Mange verbal complaints informing the
senior nursing staff and refer on when unable to resolve.
To take responsibility for timely and
accurate record keeping.
Support the assessment of patient care
needs through data collection, monitoring of BCYP progress, acting on findings
and reporting as necessary to the Registered Nurse.
To refer BCYP to other practitioners
when their needs are beyond their own role or scope of practice.
To work in partnership with parents to
plan and deliver care for BCYP. Encourage holistic development and stimulate
parents awareness, through health education and advice.
Promote the individual needs of the
BCYP, acknowledging their personal preferences, rights and choices according to
their plan of care by promoting anti-discriminatory practice, privacy, dignity,
and respect for personal values and identity
Work in an effective and organised
manner demonstrating excellent time management and organisational skills to
effectively deliver person-centered care for an allocated group of individuals.
Clinical Governance
Participate in audit activities being
undertaken in area of practice
Contribute to the improvement of
service by reflecting on own practice and supporting that of others
Adhere to legislation, policies,
procedures and guidelines both local and national Regularly attend workplace
and staff engagement meetings and contribute positively to discussions about
the improvement of care
Work in partnership with your line
manager to develop and deliver on Specific, Measurable, Achievable, Relevant
and Time-bound (SMART) objectives at annual appraisal and personal development
planning meeting
Take responsibility for organising and
attending statutory / mandatory updates in accordance with organisational
requirements
Health
and Safety:
To report and record all accidents and
incidents in line with hospice procedure safeguarding the welfare and safety
during working hours with regard to the Health and Safety at Work Act.
Ensure health and safety requirements
in relation to the provision of care to BCYP and families are managed correctly
through the provision of appropriate risk assessments.
Responsible for the provision,
monitoring, storage and safe use of equipment required to support the needs of
BCYP whilst at Derian House. This will include the training and supervision of
care staff in its use and timely reporting of any faulty equipment.
Take reasonable care to avoid injury
to self and others and to co-operate with Derian House in meeting its statutory
requirements.
To share responsibility in-house in
order to maintain a safe but homely environment taking into account health and
safety, including infection control and risk assessments.
To be able to undertake the mental,
physical and emotional demands of the role, whilst at the same time taking care
to safeguard their own health and safety as well as fellow care team members,
BCYP and their family.
Adhere
to the prevention and control of infections code of practice (Health &
Social Care Act 2008) and Derian House Childrens Hospice policies and
procedures to prevent and control infection.
