Job summary

We are looking for Registered Nurse Associatesto join our amazing team.

You will work as part of an integrated team, alongside a wider multi-disciplinary team respecting the skills, expertise and contributions of others in the provision of holistic care and support to Babies, Children, Young People (BCYP) and their families using Derian House.

We have new and exciting opportunitiesto work with ourteam who are committed to delivering the highest possible standards of respite, palliative and end of life care to babies, children and young people. Working at Derian House will involve working with a small team of dedicated professionals at all levels and also provides the opportunity to make a real difference to the babies, children and young people we support and their families.

We actively encourage ongoing professional development and have an in-house clinical education team working within a dedicated clinical skills lab to enhance and improve training and competencies.

Main duties of the job

Deliver high quality, compassionate care under the direction of a Registered Nurse with a focus on promoting high quality palliative and end of life care.

Have proficient attitudes and behaviours compatible with the values of Derian House Childrens Hospice

Work as part of the Integrated Care Team delivering care that focuses on the direct needs of the individual

Carry out specific delegated clinical and care tasks and responsibilities to a high standard and competency.

To work with the multi-disciplinary team and registered nursing staff to contribute to the ongoing assessment of patients, providing and monitoring care in line with scope of practice.

Work within the Nursing and Midwiferys Councils Code of Conduct meeting the agreed standards of proficiency.

Derian House Chidrens Hospice is rated Outstanding by the CQC; Derian House provides symptom management, palliative and end-of-life care, as well as short break respite care to babies, children, young people and their families with life limiting conditions across Lancashire, South Cumbria and Greater Manchester. Professional

Contribute to the development of a culture that is committed to the continuous improvement of quality care and patient experience.

To take responsibility for maintaining own personal and professional development, meeting with the line manager for regular supervision, annual appraisals and identifying/agreeing personal targets and education needs.

Ensure that personal actions and conduct comply with hospice policies and procedures.

To maintain confidentiality in all aspects of the role, notably information regarding the babies, children, young people (BCYP) and their families, staff and the organisation itself. Disclosure to any unauthorised person is considered a disciplinary offence.

Foster a culture of respect and consideration between all team members, respecting and nurturing the strengths, skills and knowledge of others as well as sharing own knowledge and skills.

To attend regular meetings with the multi-disciplinary team to discuss individual babies, children, and young people respecting and inviting the sharing of skills and expertise.

Maintain relevant, accurate and contemporaneous records and reports, respecting and maintaining confidentiality at all times in accordance with NMC Guidelines, Derian House Childrens Hospices policies & procedures and the Data Protection Act (1998).

Clinical

Work as part of the Integrated Care Team delivering care that focuses on the direct needs of the individual and their family.

Support people to improve and maintain their mental, physical, behavioural health and wellbeing, promoting health and preventing ill health.

Develop an understanding of all elements of the nursing process and be able to assist the registered nurse in the ongoing assessment, planning, management and evaluation of care.

To recognise changes in a BCYPs condition which require the intervention of others and understand when to refer to the supervising registered nurse and other senior colleagues for reassessment of patient care.

To perform procedures as detailed within the NMC Standards for proficiency for Nurse Associate (2018) (Annexe B) to meet the nursing needs of the children/young people in their care.

To work within the NMC code of professional practice, and within policies, procedures and guidelines of Derian House Childrens Hospice (DHCH)

To have a willingness to undertake additional skills and training dependent on the organisations requirements.

To ensure that high standards of nursing care are given and maintained. To act and raise concerns when standards are not being maintained following DHCH policy and procedures.

To act as an advocate for the BCYP and the family, including promoting and protecting their interests, privacy and rights, equality and inclusion.

To be involved in DHCH initiatives to improve patient safety and care.

Recognise issues relating to safeguarding vulnerable BCYP and report any problems or raise concerns to the appropriate registered care professionals

Assist in the assessment of and contribute to the management of risk across several areas within the environment where care is being administered

Assist in the delivery of complex care as prescribed by the registered nurse

Develop understanding of caring for individuals with particular conditions for example; complex health needs, learning disabilities, palliative and end of life care needs

Develop skills in relation to coaching/teaching individuals/carers/other staff

Assist with the implementation and monitoring of clinical standards and outcomes

Develop a working knowledge of other providers resources and referral systems to ensure individuals needs are met, within parameters of practice

Leadership and Management

To support the Registered Nurses in the day to day operational management of the service by providing leadership and supervision for Clinical Support Workers and Students.

Be resilient to manage change within the organisation

To recognise, prevent where possible and manage areas of conflict reporting to senior staff if unable to resolve.

Mange verbal complaints informing the senior nursing staff and refer on when unable to resolve.

To take responsibility for timely and accurate record keeping.

Support the assessment of patient care needs through data collection, monitoring of BCYP progress, acting on findings and reporting as necessary to the Registered Nurse.

To refer BCYP to other practitioners when their needs are beyond their own role or scope of practice.

To work in partnership with parents to plan and deliver care for BCYP. Encourage holistic development and stimulate parents awareness, through health education and advice.

Promote the individual needs of the BCYP, acknowledging their personal preferences, rights and choices according to their plan of care by promoting anti-discriminatory practice, privacy, dignity, and respect for personal values and identity

Responsible for the provision, monitoring, storage and safe use of equipment required to support the needs of BCYP whilst at Derian House. This will include the training and supervision of care staff in its use and timely reporting of any faulty equipment.

Take reasonable care to avoid injury to self and others and to co-operate with Derian House in meeting its statutory requirements.

To share responsibility in-house in order to maintain a safe but homely environment taking into account health and safety, including infection control and risk assessments.
