Job summary

We are looking for Registered Nurses to join our amazing team.

We have new and exciting opportunitiesto work with ourteam who are committed to delivering the highest possible standards of respite, palliative and end of life care to babies, children and young people. Working at Derian House will involve working with a small team of dedicated professionals at all levels and also provides the opportunity to make a real difference to the babies, children and young people we support and their families.

We actively encourage ongoing professional development and have an in-house clinical education team working within a dedicated clinical skills lab to enhance and improve training and competencies.

Main duties of the job

You will work as part of an integrated team, alongside a wider multi-disciplinary team respecting the skills, expertise and contributions of others in the provision of holistic care and support to Babies, Children, Young People (BCYP) and their families using Derian House.

You will be responsible for the assessment of the BCYPs care needs, including the development, implementation and evaluation of programmes of care.

You will assist in the management and organisation of work as required by the Care Team Leader, ensuring effective communication is maintained at all times and to provide support for junior staff within the hospice team.

Main Duties and Responsibilities:

Managerial

To support the team leader in the day to day operational management of the hospice by providing leadership and supervision for clinical support workers and students.

To participate in the orientation of new staff, students and volunteers. Acting as mentor as and when required.

Clinical

To act as a positive role model by using evidence/research based care and practice.

To be proactive in identifying ways of improving practice. Contributing positively to the development and audit of standards.

Provide and oversee the delivery of high quality, clinically effective and evidenced based nursing care and practice in accordance with Derian House Childrens Hospices policies, standard operating procedures and guidelines.

Ensure that care for babies, children and young adults is person centered, assessed, planned, delivered and evaluated respecting babies, children and young peoples daily routines and promoting independence as far as possible.

To be responsible for providing safe and effective care in line with the hospices operating procedures and guidelines and act as a resource for all members of the multi-disciplinary team providing specialist professional, care and support.

To be responsible for improving and maintaining the physical and mental health of the babies, children and young people. Ensure barriers to achieving this are reduced wherever possible and appropriate.

To adapt the delivery of care according to the emotional, physical and environmental demands.

To participate in the primary assessment for newly referred children.

To refer to the Team Leader for advice/guidance during a shift as and when required.

To be instrumental in improving the well-being of the babies, children and young people and ensure certain rights are provided to all individuals within the provision of their care.

Ensure the respect, dignity and rights of babies, children, young people and their families are upheld at all times.

To be competent in Derian Houses core and clinical competencies and individual care needs.

To be responsible and accountable for the safe administration of medicines within the hospice on a shift basis ensuring medicine management policies and procedures are adhered to as per NMC guidelines.

Clinical/Quality Assurance

Ensure the respect, dignity and rights of babies, children, young adults and their families are upheld at all times.

To undertake training as a supplementary and independent prescriber adhering to NMC Guidelines, if requested by the organisation.

To take an active role in the pursuit and promotion of quality through audit and research and participating in the hospice governance programme.

To be responsible and accountable for the safe administration of medicines within the hospice on a shift basis ensuring medicine management policies and procedures are adhered to.

To comply and ensure that non-registered staff are working within the manual handling policies and procedures.

Contribute to the day to day management of quality assurance issues reporting and recording incidents, responding to complaints and feedback in line with Derian House policy and standard operating procedures.

Support the organisation by conducting risk assessments for babies, children and young people within the hospice, reporting and recording incidents, accidents and near misses, actively participating in any corrective action required to minimise future risk, as requested.

Contribute to the safeguarding of babies, children & young adults, ensuring practice complies with Derian House Childrens Hospices and identifying any areas of concern and reporting them in a timely way to the Social Worker or a senior member of the care management team.

Communication

Regularly attend team and other meetings as appropriate, contributing positively to discussions and debates and participating in shared decision making.

Ensure effective communication including the dissemination of information accurately and timely to support effective team working.

Work collaboratively within multi-disciplinary/multi-agency teams both internally and externally to Derian House through the establishment of effective communication systems to support the delivery of high quality care.

Act at all times as an ambassador for the organisation and maintain positive and effective communication both internally and externally in representing the service offered by the hospice.

To identify the need for pre bereavement care and support working in partnership with the Family Support Team on a shift basis.

Key Working Relationships

Key working relationships include outside agencies from health, education and social care involved with the care and support of children/young people and their families.

Professional

Act at all times in accordance with relevant legal, ethical and professional legislation and frameworks (i.e. Children Act, NMC Code of Professional Conduct).

To take responsibility for maintaining own professional development, meeting with the team leader for regular supervision, annual appraisals and identifying/agreeing personal targets.

To undertake further education with the aim of potentially working towards a degree pathway if required.

Ensure that personal actions and conduct comply with hospice policies and procedures.

Ensure health and safety requirements in relation to the provision of care to babies, children, young people and families are managed correctly through the provision of appropriate risk assessments.

Responsible for the provision, monitoring, storage and safe use of equipment required to support the needs of babies, children & young people whilst at Derian House. This will include the training and supervision of care staff in its use and timely reporting of any faulty equipment.

Take reasonable care to avoid injury to yourself or to others and to co-operate with Derian House in meeting its statutory requirements.

To share responsibility within the hospice in order to maintain a safe but homely environment taking into account health and safety, including infection control and risk assessments.
