An exciting opportunity has arisen for a Advanced Nurse Practitioner to develop a pioneering role within primary care. Working as part of a team across forward thinking practices within a Primary Care Network, you will work closely with GPs, Paramedics, Clinical Pharmacists, First Contact Physiotherapists and Social Prescribers as part of a multi-disciplinary team.
The role will focus on Primary Care Network initiatives as set out in the Primary Care Network contract, NHS Long Term Plan and deliverables agreed by the network members to improve place based patient care.

Main duties of the job
The post holder will play an integral role within the Primary Care Networks and will be involved in shaping new and innovative models of care, to promote integrated working in line with the requirements of the Network Contract Directed Enhanced Service.
This role will form part of the Primary Care Multidisciplinary Team, supporting monthly neighbourhood meetings with other providers and stakeholders to improve patient pathways.

About us
Skelmersdale Primary Care Network have 6 member practices and covers a population of 55,000 patients. The GP practices that you will support in this role are: Ashurst Primary Care, Excel Primary Care, Dr Bisarya & Partners, Manor Primary Care, Beacon Primary Care and Hall Green Surgery.
Employment will be with OWLS CIC Ltd West Lancashire GP Federation, as a central function to the Primary Care Network (PCN) members. Direct line management and supervision will be provided by the Clinical Director and an assigned mentor. Support will be provided by the Primary Care Network (PCN) Manager and wider team of GPs, Practice Managers and Advanced Nurse Practitioners.

Be responsible for telephone triage of patients

Undertake Unscheduled Care clinics for patients who need to be
seen on the day

Undertake home visits as necessary

Practice in compliance with respective code of professional
conduct and within own scope of practice, being responsible and accountable for
decisions and actions

Clinically examine, assess, diagnose, plan, implement and
evaluate treatment/ interventions and care for patients, using expertise and
decision-making skills to inform clinical reasoning approaches when dealing
with differentiated and undifferentiated individual presentations and complex
situations; using information from multiple sources to make appropriate,
evidence-based judgements and/or diagnoses

Diagnose and manage both acute and chronic conditions,
initiating, evaluating, and modifying a range of interventions which may
include prescribing and reviewing medicines for therapeutic effectiveness
(appropriate to patient need and in accordance with evidence-based practice and
national protocols within scope of practice), therapies, lifestyle advice and
care into a management plan

using a range of assessment methods as appropriate (e.g., of history-taking;
holistic assessment; identifying risk factors; mental health assessments;
requesting, undertaking and/or interpreting diagnostic tests; and conducting
