Job summary

Rainbows Hospice is currently seeking a Nursing Associate Band 4 to to support the Lead Nurse in providing an expert and comprehensive hospice at home service to enhance the experience of babies, children, young people and families requiring palliative and end of life care in the home.

Location: Based at Rainbows Hospice with the requirement to travel across the East Midlands, covering Lincolnshire, Northamptonshire, Nottinghamshire, Leicestershire and Derbyshire. A driving license is required for this role.

Hours of work: Monday to Sunday 8.00am to 6.00pm. Please note flexibility is required to meet the needs of families, including occasional work outside normal hours such as staying on shift due to end of life needs.

Main duties of the job

As a hospice at home nursing associate, you will work as part of Rainbows hospice at home team providing clinical care to babies, children and young people with life threatening/life limiting conditions within their own homes, to give parents/carers care and support. The role includes assessment within your scope of practice, and providing specific, planned day-to-day care to the babies children and young people according to their level of need during Hospice at home sessions. This will involve duties which cover 7 days per week.

Here at Rainbows Childrens Hospice, we provide specialist palliative care and end-of-life support to over 750 Babies, Children, and Young People living with life-limiting conditions and terminal illnesses as well as approximately 3,000 people, including families, siblings, and carers, across the East Midlands. We are seeking a Band 4 Nursing Associate to support the Lead Nurse in delivering an expert Hospice at Home service, enhancing the experience of babies, children, young people and families requiring palliative and end of life care.

Key responsibilities include

  • Working in children and young peoples homes as part of the Hospice at Home service.

  • Supporting the Registered Nurse in delivering safe, person centred care, often working independently with an agreed plan of care.

  • Delivering excellent short break, palliative and end of life care, including care after death.

  • Participating in assessment, planning, implementation and evaluation of care under the direction of the Registered Nurse.

  • Engaging with families to understand needs, preferences and choices, contributing to risk assessments and care planning.

  • Providing a high standard of holistic care.

  • Adhering to the NMC Code and working within your scope of practice.

  • Taking responsibility for medication administration, storage and return in line with policy.

  • Maintaining accurate, contemporaneous records and handling information in line with legislation and policy.

  • Communicating sensitive information effectively and using appropriate strategies to support person centred care, equality and diversity.

  • Using SystmOne appropriately.

  • Acting independently and collaboratively to uphold the rights of individuals.

  • Liaising with the wider multidisciplinary team and external agencies to ensure continuity of care.

  • Maintaining confidentiality at all times.

  • Nursing Associate foundation degree qualification.
  • 1 year post registration experience
  • Previous experience with children and Young People aged 0-19 with complex needs
