Job summary
St
Leonard's Hospice, York is seeking to recruit a Hospice Practitioner to provide
medical care for the 20-bedded in-patient unit and support to the
community-facing Hospice@Home and Sunflower Centre teams. Based in the hospice, the successful
applicant will work with the existing team of highly experienced Hospice Practitioners, Specialty Trainees and Medical Director to ensure patients
receive responsive and high quality care.

Main duties of the job
This post is to replace the existing postholder who will retire in Spring 2021.
Applicants may be either doctors with post-registration experience, or an Advanced Clinical Practitioner (ACP) with post-qualification clinical experience. Experience within palliative care would be an advantage however your overriding quality will be that you are committed to putting our patients and their families at the centre of all clinical decision making.
Evidence of working with a multi-disciplinary team and using excellent communication skills are key, alongside experience in clinical governance, education and advance care planning. A willingness to be involved in all aspects of the Hospice and its work is also desired.
Close communication and co-ordination of care with other professionals is an essential part of this job.

The
hospice medical team are highly experienced, respected and long serving Hospice
Practitioners and Consultants. The team is complemented by rotational Specialty
Trainee posts in palliative medicine and general practice. The Hospice often
has other medical staff attachments and medical students on placement. The team work closely with the specialist
palliative care teams in York hospital and community, alongside other secondary
care professionals such as oncology and pain management.
The
Hospice is committed to continuous professional development, and will
grant appropriate study leave with funding for this purpose whenever
possible.

Clinical
- To

multi-professional team
- To
provide medical input as required to the Sunflower Centre (Day Hospice)
patients at St Leonard's Hospice
- To
provide support and advice to community and hospital palliative care teams
and other professionals.
- To
support and advise any medical colleagues who are more junior/less
experienced in palliative medicine
- Liaise
with other healthcare professionals to plan and deliver treatment in keeping with the patients wishes.
- To
participate in the non-resident on-call rota
- To
participate actively in multi-professional discussions and meetings
- To
maintain and administer medical records as per Hospice policy
- To
ensure confidentiality of information is preserved at all times, whether
at, or away from work
- To
be involved in audit, research and quality improvement programs as
appropriate
- To
help ensure that the high standard of medical care and support for carers
and families at the Hospice is maintained
Professional / Management
- To
work closely with the Medical Director to ensure provision of medical
input throughout the Hospice
- To
support the development of clinical governance, including evidence of a
commitment to clinical audit, risk management & staff development, within
the Hospice governance framework
- To
participate in the annual appraisal system for medical staff via York
Teaching Hospitals Trust, or through an existing Responsible Officer (by
agreement). For ACPs this will be
internal annual appraisal and Revalidation through the NMC or other
governing body.
- To read,
understand and follow all Hospice policies and procedures.
- To positively

to maintain the philosophy, ethos and ambience.
Education
- Attend all
mandatory training or courses
- To
participate in internal education sessions (attending and delivering) and
attend external study days as appropriate
- To
assist in delivering undergraduate teaching for medical student placements
in the hospice
Additional Information
- The
post-holder must participate in the on-call rota. The on-call commitment
is to St Leonards Hospice Inpatient Unit.
When on-call, the post-holder will have consultant cover. On call is currently 1 in 6 and is
non-resident.
- Prospective Cover Doctors
provide prospective cover for each others leave. Additional weekday and on-call sessions
worked to back-fill other forms of leave attracts sessional payment at the
usual rate.
- This
post will provide adequate medical staffing levels such that Hospice
Doctors will be expected to contribute to indirect care (for example, via
audit, writing and reviewing policies & procedures, guideline
development, teaching, governance).
Study & Training
- The
Hospice is committed to continuous professional development, and will
grant appropriate study leave with funding for this purpose whenever
possible.
- The
hospice has access to online learning through York Teaching Hospitals NHS
Trust and via Relias e-learning modules. Multi-disciplinary
education sessions are also regularly organised at the Hospice.
