If you are an experienced healthcare assistant, looking for a flexible and rewarding role, this might be the perfect position for you. You will work autonomously, predominantly in the patients home, but you will also be an integral member of the wider, multi-disciplinary team.

The post holder will provide direct personal care, primarily in the patient's home.

- Essential criteria
- Knowledge of palliative care issues An understanding of multi-disciplinary working and working with other organisations e.g. District Nurse Teams. Enhanced communication skills to work with patients, families and health/social care professionals. Ability to work in a complex environment, often in isolation and demonstrate effective coping mechanisms. Quality focused with an innovative approach to solving complex problems. Diplomacy sensitive to the needs of those affected by life-limiting illness. Flexibility to work various shifts patterns over 7 days a week. Ability to respond to changing work shifts. Must be able to travel autonomously across the geographical area covered by the service.

- IT Skills-knowledge of patient information systems e.g. EMIS
