Job summary

Hospice at Home an independent charitable organisation; is looking to recruit to a flexible part time Patient Co-ordinator and admin support worker on 15 hours per week. The post holder will be able to work flexibly to meet the needs of the Nursing Coordination Team.

Main duties of the job

The post holder will be responsible for accepting referrals to the nursing service, allocation of patient care and providing general admin support to the clinical teams. The post will also require the individual to communicate with a variety of Health and Social Care Professionals, patients/carers and members of the general public on a daily basis.

Hospice at Homes core service is our skilled team of Registered Nurses and Healthcare Assistants who provide exceptional care and support in peoples last year of life, at end of life, also providing additional services such as Lymphoedema, Bereavement and Family Support, Befriending, Occupational Therapy and Complementary Therapy.

We provide individualised care and support to improve quality of life and our end of life care helps facilitate a peaceful and dignified death in the home and support to those who are bereaved.
