Job summary

We are looking for two support workers to join our new team supporting stroke survivors and carers.

Do you have great listening and communication skills and an interest in supporting stroke survivors and their carers?

We are looking for two highly motivated and committed First Contact Stroke Support Workers to join our new team in Brighton to support stroke survivors and carers and ensure they get the right care at the right time.

Here is running a pilot Stroke Catalyst project supporting the implementation of the Integrated Community Stroke Service in Brighton & Hove in partnership with NHS Sussex, Sussex Integrated Stroke Delivery Network and the voluntary sector. The project aims to transform stroke services and enable new ways of supporting stroke survivors and carers.

You will be at the forefront of supporting Here to deliver a new stroke project,we encourage applications from anyone with an interest in improving healthcare services and delivering personalised care. We welcome applications from those looking for secondment opportunities.

Main duties of the job

In this role you will:

  1. Be the first point of contact for our patients and their friends and family, providing personalised and having meaningful conversations about what matters most to them.
  2. Work proactively to support people, and ensure our patients are connected to stroke services and people who are best placed to meet the need.
  3. Provide high quality stroke support advice & guidance.
  4. Work with NHS partners to help to develop integrated care plans for each of our patients.

First Contact Stroke Support Workers (SSW) are the first point of contact our patients, who will support stroke survivors and carers, seeking to understand their needs, and ensure they get the right care at the right time.

You will be a key contact with people being held by the service and will be required to work proactively with them to support connection and navigation of the wider healthcare system.

You will work closely with a range of stroke services and providers across Brighton & Hove and HWLH and support patients to have a smooth experience moving through the pathway.

You will work closely with NHS colleagues across the stroke pathway to develop integrated health and wellbeing plans for each of our patients. Personalised care and support planning is shaped around the needs of the person as an individual and their wider support network. At all times, patients will be at the heart of these conversations.

You will be part of an extended clinical multidisciplinary team. Both the clinical and support staff are supported by Patient Partners who support all pathways to functions and are also a key contact for the people we serve.

  • Experience of working with statutory and voluntary agencies
  • Knowledge of the NHS and different parts of the health and care system
  • Experience of record keeping and report writing
  • An understanding of the need for client confidentiality

Desirable

  • An understanding of stroke
  • An understanding of the needs of stroke survivors and their carers
  • Experience of managing volunteers

Personal attributes/ qualities

Essential

  • Able to assess and evaluate client need
  • Able to represent the needs of the organisation and clients externally
  • Able to travel independently within the service area
  • Empathetic
  • Non-judgemental communication
  • Commitment to, and understanding of, equal opportunities
  • Understanding of the inclusion agenda and its relevance within a diverse society
