Job summary

We are looking for a highly motivated, effective communicator to take up a new and exciting role as a High Intensity User Co-ordinator, which will provide the post holder the opportunity to develop networks and pathways across mental health services both within statutory and VCSE sectors. The post holder will have a range of responsibilities focused at both organisational and individual service user level.

Main duties of the job

To work with statutory and VCSE sector colleagues, and individuals with high intensity use (HIU) of healthcare. To discover underlying reasons that may be contributing to elevated health care contacts. To develop an understanding of system wide responses to HIU and identify areas for change or improvement. To support the transformation of culture, behaviours and attitudes of high intensity use of health care.

Hours: 37.5 hours per week Salary: £28,500 per annum, with progression to £31,500 per annum Location: Exeter Mooring, Wonford House, Dryden Road, EX2 5AF Fixed term contract to 31st March 2025 We are looking for a highly motivated, effective communicator to take up a new and exciting role as a High Intensity User Co-ordinator, which will provide the post holder the opportunity to develop networks and pathways across mental health services both within statutory and VCSE sectors. The post holder will have a range of responsibilities focused at both organisational and individual service user level, developing an understanding of the needs of high intensity service users and identifying gaps in service provision, making recommendations for change and improvements. You will be closely integrated with the Exeter Mooring staff team. The Mooring is an open access service offering emotional and mental health support to anyone 18+ and offers a service across day and evening/weekend hours. There is a requirement for some flexibility in working hours within this post as some evening/weekend work will be needed to enable all aspects of the role to be fulfilled.
