Job summary

We are looking for an ambitious and forward-thinking Advanced Clinical Practitioner to join our dynamic Primary Care Network. This is an exciting opportunity to shape and deliver high-quality, autonomous clinical care across our PCN, making a real difference to patients with complex, undifferentiated and long-term conditions.

The post-holder will support delivery of Network Contract DES priorities, particularly Enhanced Health in Care Homes (EHCH), anticipatory care and frailty services. This includes proactive identification and management of high-risk patients, comprehensive assessment, personalised care planning and coordinated multidisciplinary working.
