Job summary

Imp is seeking a highly motivated enthusiastic Manager to join the team and be part of the growth and development of Imp PCN in this changing landscape of primary care as we move towards system change to help lead and shape our future. The following elements of the job are expected along with flexibility to meet the needs of the organisation as they present themselves in this challenging role.

The post-holder will work with clinical directors (of Imp PCN & Imp Healthcare Ltd) and the leadership within a Primary Care Network (PCN) to create a vision to guide service improvements, identify opportunities to make tangible positive transformation, and provide the inspiration, expertise, and leadership to deliver changes in a collaborative way.

The post-holder will support this vision to be developed in alignment with ICS strategy.

Main duties of the job

The post-holder will plan, co-ordinate and directly support the delivery of transformation activity in general practice within a PCN with a view to improving patient access, quality and continuity of care, staff and patient satisfaction, and the efficiency and sustainability of general practice services.

The post-holder will work closely with staff at the 9 PCN practices, such as practice managers and staff at the PCN to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered.

The post-holder will develop strong relationships with their local PCN practices and will support the planning and delivery of improvements that will provide benefits to patients and staff. The post-holder will develop relationships with the ICS to discuss and agree local transformation plans and activity and to support alignment with broader strategy, including the ICSs digital strategy.

Working with PCN leadership, the post-holder will help to develop strategy related to transformation and improvement, aligning this with larger scale strategy and initiatives, particularly at ICS level. They will contribute to collaborative problem solving with other system partners, patients and communities, focusing their time on the planning, implementation and evaluation of practical improvements that make a difference to patients and staff in the PCN.

Strategy and Leadership

Work with the PCN leadership team to design, plan and implement the PCN vision and strategy.

Developing, implementing and embedding an efficient PCN resilience plan

Represent the PCN within the place area and wider Integrated Care System (ICS)

Organisation of PCN meetings for internal and external attendees; scheduling meetings, inviting relevant parties, maintain and proactively chase the conclusion of actions arising.

Liaising at external meetings as required

Work in partnership with other local health and care organisations within the area on joint initiatives and service improvement projects

Act as the primary point of contact for NHSE, ICB, community services and other external stakeholders

Seek and encourage ideas, projects and improvements from across the organisation.

Working closely with Imp PCN Clinical Leads and supporting the PCN team in new areas of work.

Lead on commissioning of services (direct commissioning and sub-contracting) for the PCN.

HR

Employed Staff

Personnel functions for all employed staff recruitment, induction, discipline, retirement, personal, legal, leave entitlement, sickness etc. including but limited to pre-employment check, initial DBS check and annual DBS declaration, obtaining of references, issuing of contracts, management of sickness including return to work interviews, approving of annual leave requests, obtaining of windows accounts, NHS emails and smartcards.

Prepare and circulate the completed pre-employment form for all staff to all PCN practices prior to their commencement date in the PCN and practices.

Ensure that details of any leavers are circulated to practices to ensure that SystmOne access is removed.

Order uniform as required.

To ensure that all staff are appropriately vaccinated and maintaining the PCN/ARRS register of staff vaccinations.

Ensuring registered professionals, maintain their registration and indemnity and maintaining the PCN/ARRS register for this.

Co-ordinating clinical supervision and recording dates when meetings/supervision has occurred.

Co-ordinating clinical audits and recording dates of audits, findings and follow up discussions when applicable.

Ensure that details of any leavers are circulated to practices to ensure that SystmOne access is removed.

To ensure that all staff are appropriately vaccinated and maintaining the PCN/ARRS register of staff vaccinations.

Support the Board and practice teams to ensure understanding of the current PCN guidance from NHS England and the Lincolnshire Integrated Care Board (LICB), keeping them informed of anything new or significant.

Coordinate and liaise with the Extended Board members including arrangements for meeting attendance. Act as a point of contact for all stakeholders (PPG, Healthwatch, ICCT).

Ensure Board meetings are run in accordance with the Terms of Reference of the PCN Board.

Approve the board minutes before circulating to the Board members.

As required, to minute the PCN Board meeting, and other meetings.

Digital and Transformation

Lead on change management for the PCN, using formal and informal change methods.

Identify digital technology that can improve service delivery and patient outcomes for the PCN.

Formulate and present business cases for the purchase and/or use of this technology.

Produce management information packs for PCN Board and PCN projects.

Develop high-level understanding of capacity and access data sets and technology.

Develop high-level proficiency with SNOMED codes and business rules.
