Main duties of the job
The Digital Health Service provides a mechanism for delivering 21st-century care and connecting people with medical and non-medical sources of support, empowering people to better self-manage their conditions and/or work collaboratively with healthcare professionals to manage their conditions.
The Digital Health Service delivers expert clinical case management using virtual technologies. Patients with shortand long-term conditions may be at risk of deteriorating health, which can result in declining quality of life or avoidable hospital admission.
The Digital Health Team Leader is an essential supervisory role for a service monitoring and responding to markers of declining patient health. Proactive and flexible team management enables team members to deliver the best care possible to patients on the Digital Health Service.
The Digital Health Team Leader is responsible for the general day to day running of EBPC Digital Health team members, Nurses and AHPs working both within the EBPC call centre or remotely.
The expectation is to monitor the work of Nurses, AHPs, Care Navigators, and Clinical Support team members, ensure shifts are covered (by liaising with the Resources Department), and ensure staff can use Docobo and Connected Care correctly. This will involve an element of training and one-to-one assistance to ensure all Digital Health team members are up to date with Docobo and other systems.

Job responsibilities
The post holder will:
- Supervise Nurses, AHPs, Care Navigators and Clinical Support team members in the use of Docobo, Connected Care, Storm, EMIS and Adastra; ensuring their knowledge of Remote Monitoring Standard Operating Procedures (SOPs) is complete and accurate.
- Ensure all shifts are covered and rotas amended when required.
- Schedule daily tasks for the team based on service needs.
- Manage annual leave and sickness for the Digital Health team members and act as a point of contact for issues regarding this, which may need to be escalated.
- Liaising with Docobo on updates and current versions then passing on these updates and changes to the Digital Health Team so that they stay completely up to date. This may involve attending training days or meetings when required.
- Provide training for Digital Health members on the use of Docobo, Connected Care, Storm, MS Teams, Message Media, Adastra and EMIS when required.
- Ensure Digital Health team members have regular one to one meetings, annual appraisals, and regular auditing/monitoring of calls using STORM data. Call audits with Nurses and AHPs will be reviewed with the Clinical Operations Manager, when required.
- Ensure regular Digital Health team meetings and engagement with a focus on governance and learning throughout the agenda.
- Work with partners in nursing and residential care homes to ensure optimum health outcomes for their residents.
- Working with GP practices for non-urgent patient escalation/reviews.
- Work in alignment with Primary Care, for established practice populations, toward a shared vision of seamless management for patients with short and long-term conditions within the Primary Care and the Community setting.
- Work with ICS partners in Primary and Social care to further develop the Digital Health model for cohorts of patients managed within local populations.
- Assist in the compilation of processes and procedures for any new Digital Health services.
- Co-ordinating alerts for patients from the out of hours period; subsequently, populating outcomes from out of hours care onto the patients Docobo Digital Health record.
- Collating patient enrolment/discharge data and statistics for GP feedback.
- Assist in the continuous updating of SOPs, manuals and handbooks as necessary so that they remain consistent and up to date for the Digital Health teams use.
- Understand and support the achievement of EBPCs business plan objectives and performance targets.
- Perform other ad hoc tasks as required appropriate to the role.
- Have experience and competence using other clinical patient management systems: Adastra, EMIS, Connected Care.
RESPONSIBILITIES FOR PEOPLE AND TRAINING
- Act as expert resource and assist in teaching competent use of the Docobo clinical record and Digital Health system.
- Take part in the process of clinical supervision and participate in clinical supervision and mentorship of others.
- Provide patients, families and carers with tailored advice and support to increase understanding of signs and potential causes for acute exacerbation of underlying conditions. This may aid patient, family and carers engagement with lifestyle changes that would be advantageous to health.
- Be responsible for providing and maintaining a learning environment and maximise opportunities for education and development in the clinical area to enhance individual development and performance in the delivery of high standards of care.
- Take responsibility for your own personal and professional development; maintain competence, knowledge, and skills commensurate with role.
- To be actively involved in clinical staff recruitment in the Digital Health Team when required by EBPC.
