Job summary
The PCN Pharmacy Care Coordinator will play a key role, complementing clinical pharmacists, pharmacy technicians and other members of the PCN multi-disciplinary team, working under supervision to ensure effective and efficient use of medicines. The post holder will work in close partnership with the pharmacy team to proactively identify and deliver services and projects that support all aspects of medicines optimisation, helping maintain the health and wellbeing of the patient population. They will work with external stakeholders to bridge the gap between the practice team and other clinicians working collaboratively to ensure all aspects of medication safety are adhered too. The post-holder will develop and encourage a positive culture of medicines optimisation principles to assist the PCN in achieving local and national medicines-related objectives.

Berkshire Primary Care Ltd (BPC Ltd) is a GP Federation which consists of the practices within Bracknell & District and Ascot Primary Care Networks (PCNs). GP Federations are a way for practices to work collaboratively to improve the services they can offer to patients and to strengthen the role of primary care within the wider NHS. We are proud to be the chosen provider delivering the Extended Hours primary care services for both Bracknell and Ascot, covering approx. 140,000 patients. During the pandemic we were the key provider of covid vaccinations for the population across Bracknell and Ascot. In addition to delivering core services, we explore new and innovative approaches such as remote INR monitoring, NHS health checks, screening for atrial fibrillation, hypertension case finding and our award-winning remote monitoring for high-risk patients. We work closely with Frimley integrated care board, primary care and secondary care partners to deliver innovative, sustainable and high-quality services. Where possible we always aim to use new technologies and in novation to improve service delivery. We believe that sustainability of our services is essential for us, our practices and our healthcare partners. We strive to consistently provide a high-quality service for our patients.
