Job summary

An exciting opportunity to join our team of Paramedics who provide services to practices in Bracknell & Ascot. The scope of the role includes a home visiting service plus the vision to assist with urgent and on the day demand as support to the GPs and Practice Clinical Teams. You will be based in our central Bracknell Head Office.

Alongside Paramedics we employ a team of Clinical Pharmacists, Pharmacy Technicians, Care Coordinators and Social Prescribers as part of our "Living Well" service for our patient population.

Main duties of the job

Carry out home visits to support the GPs.

Triage of online consultations received by our Virtual Hub

Carry out and interpret diagnostic tests under supervision of a GP.

Independently decide on and carry out treatment working under the remit of local PGDs Assess and evaluate, with patients, the effectiveness of the treatment and care provided.

Refer patients for investigations and/or the appropriate specialist where appropriate Ensure the provision of continuity of care including follow-up visits.

Take a holistic approach to patient care, bringing together their medical, social and psychological needs.

Empower patients to take responsibility for managing their own care and treatment, where it is safe and appropriate to do so.

Record the Consultation in the patient records in the EMIS Clinical System.

Contribute to and write Care Plans - For those patients at risk of hospital admission, for example, in a care home or with complex needs they have a care plan which is recorded in the patients clinical record.

Job description

Job responsibilities

The Paramedic will have the following key responsibilities, in delivering health services:

a. work as part of a MDT within the PCN;

b. assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team;

c. advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the PCNs social prescribing service, and where appropriate, other community or voluntary services;

d. be able to:

i. perform specialist health checks and reviews within their scope of practice and in line with local and national guidance;

ii. perform and interpret ECGs;

iii. perform investigatory procedures as required; and

iv. undertake the collection of pathological specimens including intravenous blood samples, swabs, and other samples within their scope of practice, and within line of local and national guidance;

e. support the delivery of anticipatory care plans and lead certain community services (e.g. monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing);

f. provide an alternative model to urgent and same day GP home visit for the network and clinical audits;

g. communicate at all levels across organisations ensuring that an effective, person-centred service is delivered;

h. communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required;

i. maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice; and

j. communicate effectively with patients, and where appropriate family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings, and treatment choices.

Home Visiting role:

The BPC Home Visiting Service offers appointments for patients who cannot attend the Practice for routine appointment or who need an urgent home visit. You will work closely with clinicians from the Practices in Bracknell and Ascot from both Ascot and Bracknell & Districts Primary Care Networks

You will be responsible for providing telephone and face to face consultations for patients presenting, unscheduled, with a wide range of minor illness, long term conditions and acute healthcare needs.

You will be expected to:

Qualifications

Essential

  • You are a qualified paramedic registered with the HCPC (Health and Care Professionals Council) Member of College of paramedics.
  • You have completed your two-year Consolidation of Learning period as a newly qualified paramedic;
  • You have a further three years experience as a band 6 (or equivalent) paramedic;
  • You have a record of acting as a paramedic practitioner / urgent care practitioner dealing with patients presenting with a wide range of medical issues

Desirable

  • Background of working in a fast paced Primary Health Care or acute environments

Experience

Essential

  • Proven background of dealing with a diverse range of patients.
  • Proven background of building positive rapport with a diverse range of internal colleagues and clients.
  • Proven ability to work autonomously whilst also having an awareness when identifying the need to access the wider clinical team or external agencies appropriately and safely.
  • First class organisational and time management skills in a fast paced environment are essential.
