Job summary
We are looking for a motivated, dynamic and positive band 6 nurse with excellent communication and record keeping skills to work within the care coordination team in all Claire House settings; Wirral Hospice, Liverpool Hospice and the community setting. The individual will be caring, professional and capable of working across teams and services internally and externally.
We are looking for extensive experience and knowledge within nursing care, to include effective leadership skills and with a knowledge of safeguarding procedures. The role involves supporting families holistically within their journey at Claire House, participating and chairing meetings and is the main point of contact for a large caseload of families and professionals. To build positive relationships, gain updates, work collaboratively and provide appropriate support to families in times of need.
This role is full-time and can be worked flexibly across 4 days, minimum of 30 hours is required, predominantly office hours, with an expectation to work flexibility over a 7-day period if needed, with an option to work at home at times; dependent on service need.
Key skills for this job include: Experience working with children/young people and families, excellent communication skills, excellent organisational skills, ability to lead a small team and to work autonomously. To be able to prioritise, manage time and workload, problem solve and meet deadlines. Good IT skills and a car driver is essential.

PART 1: JOB PROFILE
1. Main Purpose of Job
To be responsible for following the journey of children/young people and their families once they have been through the referral process.
To be part of a care coordination team, overseeing the total caseload ensuring that all children receive the right service, at the right time, for the right amount of time. Acting as the main point of contact for families and professionals.
To practice and promote a high level of specialist palliative care, encompassing a supportive and holistic approach to the child/young person and their families with a life-limiting condition. The post holder is responsible as a registered, accountable and autonomous nurse to act in the best interests of the child/ young person and their family at all times. You will maximise the use of the multi-skilled team to plan, deliver, evaluate and maintain safe, compassionate and effective care.
As a Registered Practitioner, you will be required to demonstrate leadership qualities; being open, transparent and trustworthy. This is in accordance with the Nursing and Midwifery Council (NMC) Code of Practice and Claire House vision and values.
