Job summary
We have an exciting opportunity for an enthusiastic, highly motivated health and social care professional to join our Hospice in Your Home team to co-ordinate and deliver exceptional care in the community.
About the roleThe HiYH Supportive Care Co-ordinator will be responsible for the effective and efficient co-ordination of care visits for the Hospice in Your Home team, ensuring a responsive and individualised service for patients with specialist palliative care needs and those important to them.

The available role:We have 2 permanent roles available. This could be two part-time roles or one full-time role working 37 hours per week and one part-time role working 18.5 hours per week.Salary:£22,833 - £24,775 per annum

Main duties of the job
Utilising outstanding organisational skills, the post holder will develop, implement and maintain the robust systems and processes necessary for the effective co-ordination of HiYH visits. They will oversee the delivery of clinical care and associated activities in line with appropriate standards and guidelines, demonstrating care and compassion when communicating with patients and those important to them to plan appropriate individualised care.We are looking for an individual with significant experience in co-ordinating the provision of health and social care in a community setting, matching service user need to available staffing in line with identified priorities. The successful post-holder will have excellent organisational and time management skills along with excellent communication skills.

We provide a wide range of palliative care services to people with advancing life-limiting illnesses of any diagnosis and to those people important to them.
