Job summary

Are you a highly motivated skilled and compassionate social worker keen to join our multi-disciplinary community team providing palliative care to adults in Brighton & Hove, Peacehaven and Newhaven through a range of community, outpatient and in-patient services?

We are seeking someone with experience of palliative care, or else someone interested in a career move into palliative care. You will be experienced and confident in working with emotional and relational complexity and risk in a multi-disciplinary context; working alongside other colleagues in the Patient & Family Support Services team to deliver a seamless psychosocial offer to patients and their significant others.

Main duties of the job

  • To work as a member of the Patient and Family Support Service, responsible for the provision of a social work service to our patients and families in their own homes, in our inpatient unit and at our on-site (and future satellite) outpatient clinics.
  • To work within a multidisciplinary team approach contributing an understanding of the emotional, psychological and social needs of individuals and families facing death and bereavement.
  • As a member of the multidisciplinary team, the post holder will offer psychosocial interventions to patients with life limiting illnesses and those close to them, including children.
  • To provide emotional and/or practical support to patients and/or their families both pre and post bereavement.
  • To be a designated Safeguarding Lead for the organisation. Must have up to date knowledge of safeguarding legislation and procedures for adults and children. This would include advising colleagues in raising formal safeguarding concerns to the local authority. Delivering refresher safeguarding training to all clinical staff members across the service.
  • To act as Compassionate Neighbours Lead. Oversight of overall project, line managing of Project Coordinator, attending inter hospice network meetings, delivering training to new volunteers, facilitating Practice development meetings and compiling programme board reports.

  • Social work qualification CQSW/Dip/BA/MA Social Work Current registration with Social Work England Good knowledge and grounding of the issues and legal requirements around safeguarding adults at risk; Mental Capacity Act 2005; child protection Awareness of ethical and social policy issues related to death and dying Knowledge of the impact of illness and anticipatory grieving on individual and family functioning

Desirable

  • Post graduate training in family & group work Supervision of volunteers

  • Experience of working with adults with acute or chronic health problems facing major loss or change
