Job summary

MARTLETS HOSPICE, BRIGHTON invites applications for a Consultant in Palliative Medicine to join the existing team of three consultants, working in both hospice inpatient and community settings. This is a part time (7 PAs), substantive post which attracts full NHS Terms and Conditions.

Martlets clinical services include an 18 bedded specialist palliative care in-patient unit, a busy community specialist palliative care service, which includes a dedicated Hospice at Home team and a growing suite of out-patient services. In the most recent Care Quality Commission report (February 2016), the Martlets Hospice was rated Outstanding for Caring and Good overall.

Martlets is a responsive, agile and creative organisation and this has never been more apparent than during the recent pandemic. We are keen to develop all that has been learned through this time of transformation and are seeking a consultant colleague that will contribute to further growth and innovation.

Main duties of the job

The job plan comprises 7 programmed activities including a 1 in 6 second on call commitment.

The successful candidate will join the existing team of three consultants providing medical support and leadership within Martlets hospice and community settings.

The post includes a key role in medical education and training, and out-patient service development. Medical leadership to further optimise our use of digital technologies to support our clinical work would be an optional opportunity.

Applicants must have full registration with the GMC, with a current licence to practice and be on the Specialist Register, or be within 6 months of obtaining a CCT, at the time of the advisory appointments committee.

Martlets Hospice is located in Hove, East Sussex with its catchment area covering Brighton and Hove and the coastal strip up to Newhaven. The organisations clinical services include an 18 bedded specialist palliative care inpatient unit and a busy community specialist palliative care service, which consists of a multi disciplinary community palliative care team, a dedicated Hospice at Home team and a growing suite of out patient services.

Tasks and Responsibilities

To contribute to the professional and clinical supervision of junior/middle grade medical staff. This will include day to day clinical supervision of these staff when on the inpatient unit ward, as well as regular, formal 1 to 1 sessions with one or two SAS/Trust Grade doctors.

To participate in clinical audit and support the wider clinical governance agenda within Martlets Hospice, including identifying and implementing evidence based practice with appropriate guidelines and standards, and integrated care pathways.

To contribute to the education programmes run by the Martlets Hospice. This will include work with medical students, junior doctors, GPs, trained nurses from the hospice and community and nursing students, as well as other members of the multidisciplinary teams.

To support a culture of innovation, development and research in palliative care within the hospice, involving staff, and patients and their families/significant others.

To engage in the Appraisal and Revalidation system with Brighton and Sussex University Hospitals NHS Trust as well as associated job planning/objective setting meetings with Martlets Medical Director.

The post includes a key role in medical education and training, and outpatient service development. Medical leadership to further optimise our use of digital technologies to support our clinical work would be an optional opportunity. Whilst the recent pandemic has resulted in rapid and creative transformational change which has, in effect, propelled the organisation through many objectives outlined in our 2018 five year Clinical Strategy, we still aim to see major growth to ensure those with unmet needs are able to access appropriate care. The post holder should be willing and able to work with our Service User Involvement Lead to help inform strategic planning and service development.

Resources available to the Post Holder

  • A dedicated clinical administrative and secretarial team supports the consultant team.
  • The post holder will have a desk in a shared, well equipped office.
  • Information technology (IT) facilities include access to nhs.net, PACS and WinPath, Somerset database, and the Martlets intranet and electronic patient records. The IT facilities also include access and equipment to support remote working if appropriate. Necessary IT training will be provided.
  • Regular 1 to 1 sessions with the line manager (Martlets Medical Director).
  • Regular clinical supervision with an external supervisor on a 1 to 1 basis is available.
  • Monthly consultants meetings to discuss operational and professional issues and for peer support.
  • Mentoring can be put in place for a newly appointed consultant if this is identified as something the post holder would benefit from or would like.
  • Regular multiprofessional Journal Club.
  • Martlets Doctors Education Meetings, held twice a year with the GPs providing first on call cover for the out of hours rota.

Audit and Clinical Governance

  • The post holder will be expected to support and deliver all elements of good clinical governance, including risk management, clinical audit, incident reporting, and other aspects of quality improvement.

Teaching and Training

  • To provide guidance and support to immediate medical and multidisciplinary team colleagues and the wider service where appropriate, and to participate in education and training for internal and external teams in collaboration with the Hospice Organisational Development department.
  • Martlets has close links with Brighton and Sussex Medical School (BSMS). The post holder will be expected to be involved in the teaching of medical students at different stages in their training.

Research

The Martlets has a strong commitment to research and engaging in research within this post is strongly encouraged.

There is a consultant in the team who is the Research Lead.

Maintaining Good Medical Practice

  • All doctors are expected to participate in appropriate and relevant Continuing Professional Development opportunities and to remain up to date with mandatory and statutory training.
  • Commitment to Medical Appraisal and Revalidation (this is supported by BSUH).
  • Doctors are actively encouraged to take their study leave entitlement in line with the Royal College guidelines and to support the development needs identified in their Professional Development Plan, peer group reviews and appraisal.
  • There is a dedicated study leave budget and a process for approval which considers the individual Professional Development Plan.
  • The consultant team is committed to ensuring there are opportunities for learning and development and for sharing best practice across palliative care services in the local area. Examples include regular CPD meetings with senior medical colleagues from other regional palliative care teams.

On Call Commitments

  • The post holder will be required to participate in a 1 in 6 on call rota.
  • The on-call rota is covered by the four Martlets Palliative Medicine Consultants (including the post holder) and the two Palliative Medicine Consultants based at the Royal Sussex County Hospital.
  • The on call is nonresident with second on call duties and internal cover.
  • The on-call duties include provision of specialist palliative care advice to the:

Hospice inpatient unit, working with an established rota of 10 GPs who are first on call.

Community teams including Martlets Hospice Clinical Nurse Specialists and Hospice at Home Nurses, generalist Community Nurses and GPs.

Clinical staff at the Royal Sussex County Hospital, Brighton.

  • Visits to the inpatient unit may be required on occasion if deemed clinically necessary and also on bank holidays; otherwise all other clinical advice is via telephone.
  • The on-call commitments attract 1.0 PA per week (of Direct Clinical Care activity).
