Job summary

Dougie Mac are currently looking to recruit Community Clinical Nurse Specialist Team Leaders who share our passion in providing excellent, personalised and compassionate care to patients and their families within our newly restructured community team.

At Dougie Mac our vision is to be admired as a centre of excellence for palliative and end of life care. Our many different services provide patients, their families and carers, with physical, emotional, social, psychological and spiritual support, at home, or here at the Hospice.

Main duties of the job

The right candidate will possess excellent communication and organisational skills, a motivation for continual learning and sharing of knowledge, with the drive to ensure our patients receive the highest possible standard of care.

The DMH Community CNS Team is a responsive and innovative team providing care for people with progressive, incurable diseases such as cancer, motor neurone disease, heart failure and lung disease. Our community structure encompasses a Hub which operates a specialist adviceline, triage and rapid response service alongside community caseload teams aligned to Primary Care Networks. The service operates 7 days a week from 7am-9pm.

Job Summary: Working with the Head of Community services you will provide effective leadership of a self- managed community team at Douglas Macmillan Hospice. To promote and engage staff in new ways of working, such as the use of technology to ensure the timely, responsive and high-quality service is maintained and well organised for patients and their families. You'll provide professional leadership, management and expertise to the community team and wider multi professional team (internal and external). You will be responsible for creating and maintaining a sense of interdependence amongst team members and work towards a common goal. To work with the aligned Primary Care Network’s to create bespoke links and communication with GPs and other stakeholders. Engendering a positive working environment with an emphasis on professionalism, teamwork and co-operation. You will take responsibility for creating and maintaining a culture of continuous improvement, ensuring high quality standards of care are maintained. It is expected that the team leader will have a management role and it is the responsibility of the team leader to effectively manage their workload to prioritise their clinical and managerial duties.

  • - Degree in Nursing Practise
  • - Masters level of education or willingness to work towards
  • - Evidence of recent continued professional Development
  • - Nurse Prescriber or willingness to work towards
  • - Advanced communication skills qualification
