Job summary

Dougie Mac are currently looking to recruit Registered Nurses to our developing Young Adult Service. Successful candidates will share our passion in providing excellent holistic and person-centred care to young adults and their families.

At Dougie Mac our vision is to be admired as a centre of excellence for palliative care. Our new, developing Young Adult Service will provide service users and their families with physical, emotional, social, psychological and spiritual support, both within the hospice and out in the community.

Main duties of the job

To work as an integral part of the Young Adult Service: assessing, implementing and evaluating nursing care for service users living with palliative or complex health conditions.

To complete a wide range of clinical interventions.

To act as a mentor / role model to Care Support Workers.

The right candidate will possess excellent communication and interpersonal skills, with the drive to ensure our young adults and their families receive the highest possible standard of care.

Job Summary:

To be responsible for the following:

1. To work as an autonomous practitioner in assessing, implementing and evaluating nursing care to children, young people and families served by the Trust.

2. Work in partnership with other professionals and agencies to provide planned child and family focused care.

3. To act in an advisory role to other professionals and staff from other agencies in respect of palliative care for children and families.

Duties & Responsibilities:

· Train and assess competence of Care Support Workers

· Acting as a role model within team, motivating staff and setting an appropriate example

· To contribute to the training, education and support of a range of both medical and nursing students on clinical placement with the Trust.

· Co-ordinate annual leave and sickness records in the absence of Team Leader

Responsibility for patient care

· Assess, plan, implement and evaluate plans of care for children in partnership with child, parents/ carers and other professionals

· Provide effective care, treatment and management within the hospice and/or community setting

· To ensure all documentation is complete and kept up to date in accordance with Trust policy

· Liaise with healthcare and social care professionals across organisations to ensure an integrated package of care and support designed to meet the specific needs of the family, is delivered and co-coordinated.

· Ensure care delivered is delivered to predetermined standards

· Work in partnership to actively involve parents/carers and children in the implementation and evaluate of care plans.

· Provide information for children, young people and families, giving clear ongoing information as the child’s illness progresses, ensuring that parents are facilitated to maintain control and make informed decisions relating to the care of their child.

Responsibility for Resources

· Promote the safe and effective use of equipment and resources

· Inform and liaise with Team Leader in regard to resource needs, including skill mix

· Have a good knowledge of equipment available and process of appropriate equipment supply.

Strategic and service responsibilities

· Attend meetings relevant to post and on behalf of the Team Leader or Organisation

· Implement changes to established practices and/or policies

· Participate in service development under direction of Team Leader and Director of Care.

· Participate in and conduct audits of service in partnership with team leader.

· Deputise for Team Leader in their absence

· Ensure practice is clinically effective and evidence based

· Responsible for own professional development

· Contribute to the development and provision of a safe, effective and efficient service

  • Registered Childrens Nurse RNCB/ Registered Learning Disability Nurse RNLD (If RNLD it is essential to have relevant recent experience in caring for sick children or children with additional needs)

Desirable

  • ENB 998 or C&G 7307 or equivalent.
  • Childrens Palliative Care qualification
