Job summary

Dougie Mac are currently looking to recruit Care Support Workers to our developing Young Adult Service. Successful candidates will share our passion in providing excellent holistic and person-centred care to young adults and their families.

At Dougie Mac our vision is to be admired as a centre of excellence for palliative care. Our new, developing Young Adult Service will provide service users and their families with physical, emotional, social, psychological and spiritual support, both within the hospice and out in the community.

Main duties of the job

To work as part of a nurse led service caring for young adults with life limiting / life threatening conditions and/or complex health care needs.

The right candidate will possess excellent communication and interpersonal skills, with the drive to ensure our young adults and their families receive the highest possible standard of care.

This role can be full or part time and will involve working flexibly to cover each 24 hour period. Please read the job description (below) for further details.

Job responsibilities

Job Summary:

To work as an integral part of the Young Adult Service providing physical, emotional and recreational care and support to service users with life limiting / life threatening and often complex care needs.

To provide a high standard of care to young adults and their families, according to the care plan and following local and national policies and procedures.

Duties & Responsibilities:

· To implement, in conjunction with the young adult or their parents/carers, a person-centred plan of care to meet their identified needs.

· To work as part of a nurse led service caring for young adults with life limiting illnesses and/or complex health care needs, under the supervision of registered nurses

· To carry out home/community visits as deemed necessary and/or appropriate by the Team leader or Young Adult Service Lead.

· To act as a resource for other members of the multi-disciplinary team, young adults and their parents/carers

· To undertake agreed projects either individually or as part pf a team, using own initiative where necessary

· Contribute to the assessment of young adults’ health and wellbeing alongside registered nursing staff

· Monitor and evaluate health and care management alongside registered nursing staff

· Maintain and contribute to professional records, according to policy, ensuring confidentiality at all times

· Contribute to systems and processes, which support safe and effective care delivery

· Report to the shift coordinator, any concerns surrounding to the care, condition or behaviour of the young adult or family member

· Ensure effective day-to-day communication in respect of health and care management with the young adult or their parents / carers as appropriate.

· Contribute to the safeguarding of adults in accordance with local policy, procedure and guidance.

· To be responsible and observe a personal duty of care for the safe and effective use of equipment.

Training and Education

· Meet the competency requirements identified within the role and participate effectively in personal development, clinical supervision and reflective practice

· To undertake specific training and competency assessments as required to meet the needs of all service users.

· To attend team meetings, staff development programmes, training courses and study days as deemed necessary by the Young Adult Service Lead.

· To take personal responsibility for attendance at mandatory training

· To take personal responsibility for ensuring all competency assessment are completed and reassessed on an annual basis

· To take a personal responsibility for identifying any gaps in knowledge/skills and taking appropriate action to rectify this

Communications and Working Relationships

· The post holder is expected to foster excellent working relationships with all service recipients, staff, visitors and donors. In particular, they are expected to deal sensitively with all service users and their families.

· To act at all time in professional manner, respecting the needs of colleagues and co-operating to maintain a harmonious working environment

  • - Relevant NVQ level 2/3 or equivalent
  • - English and maths at GCSE level, or equivalent)

Experience

  • - Experience of working independently without direct supervision
