Job responsibilities
Job Summary:
To work as an autonomous practitioner in assessing, implementing
and evaluating nursing care to children, young people and families served by
the Trust.
Work in partnership with other professionals and agencies to
provide planned child and family focused care.
To act in an advisory role to other professionals and staff from
other agencies in respect of palliative care for children and families.
Duties
& Responsibilities:
Train and assess competence of Care Support Workers
Acting as a role model within team, motivating staff and setting
an appropriate example
To contribute to the training, education and support of a range of
both medical and nursing students on clinical placement with the Trust.
Co-ordinate annual leave and sickness records in the absence of
Team Leader
Responsibility
for patient care
Assess, plan, implement and evaluate plans of care for children in
partnership with child, parents/ carers and other professionals
Provide effective care, treatment and management within the
hospice and/or community setting
To ensure all documentation is complete and kept up to date in
accordance with Trust policy
Liaise with healthcare and social care professionals across
organisations to ensure an integrated package of care and support designed to
meet the specific needs of the family, is delivered and co-coordinated.
Ensure care delivered is delivered to predetermined standards
Work in partnership to actively involve parents/carers and
children in the implementation and evaluate of care plans.
Provide information for children, young people and families,
giving clear ongoing information as the childs illness progresses, ensuring
that parents are facilitated to maintain control and make informed decisions
relating to the care of their child.
Responsibility
for Resources
Promote the safe and effective use of equipment and resources
Inform and liaise with Team Leader in regard to resource needs,
including skill mix
Have a good knowledge of equipment available and process of
appropriate equipment supply.
Strategic
and service responsibilities
Attend meetings relevant to post and on behalf of the Team Leader
or Organisation
Implement changes to established practices and/or policies
Participate in service development under direction of Team Leader
and Director of Care.
Participate in and conduct audits of service in partnership with
team leader.
Deputise for Team Leader in their absence
Ensure practice is clinically effective and evidence based
Responsible for own professional development
Contribute to the development and provision of a safe, effective
and efficient service

Attendance at
meetings events and activities may require the post holder to work beyond the normal
hours / days of work quite regularly flexibility is essential.
To have an
understanding and demonstrate regard for the Hospice philosophy and its
functions.
Travel in the Stoke
on Trent / North Staffordshire area, and on occasions within the UK, to attend
participate in training and personal development opportunities, using own
vehicle or public transport, for which expenses will be met in line with
policies in the staff handbook.
To maintain an
effective and collaborative working relationship with other members of staff
and volunteers.
Conform to
conventional standards of business dress when representing the hospice.
To accept temporary
redeployment within any part of the hospice should the need arise and to ensure
hospice goals and objectives are met.

are expected to follow the Hospices Safeguarding Policies and report any
concerns in accordance with those policies.
Safety
Health Environment (SHE):
All employees must
familiarise themselves with the Hospice Safety Health Environment (SHE)
policies, processes and procedures and act in accordance with them at all
times. Employees must ensure that they conduct themselves in a manner which
does not endanger their own health and safety or that of others.
All employees have a duty
of care to promptly report any near misses, dangerous occurrences or accidents
whilst at work in line with the relevant Safety Health Environment procedures
All employees should attend
mandatory Safety Health Environment training sessions annually/periodically for
defined activities etc and ensure that they follow the processes and procedures
as outlined in the training
Any breaches of the Safety
Health Environment Health standards will be dealt with under the DMH
disciplinary procedure.

of care provided by every member of staff is paramount.
