Job summary

Were delighted to be hiring a Wellbeing Coordinator to join our small, supportive and successful team. We take a personalised, holistic and collaborative approach to assessing and supporting individuals referred by GPs, Foodbanks and other partnership agencies. We are commissioned by the London Borough of Wandsworth and the NHS Wandsworth Integrated Care Board to deliver much needed emotional practical support for between 2-6 sessions, in a community setting, GP surgery or by phone. The latter is likely to be the platform for engagement with service users due to the time constraints of working 1 day a week. This role will largely be office based.

Main duties of the job

Your impact Collaborating with partnership agencies and promoting co-production with guests and service users, you will provide 1:1 personalised support to individuals and identify wider issues that impact health and wellbeing. You will address individuals unique needs by designing bespoke wellbeing plans and establishing partnerships, helping individuals make positive, long-lasting lifestyle changes and play a pivotal role in diversifying and personalising care in local communities.

Your skills To join us as Wellbeing Coordinator you will have excellent holistic assessment and consultation skills. Youll be an effective communicator who excels at building trusting and empowering relationships with individuals, and be able to work closely with a variety of professionals and stakeholders. You will be a skilled and experienced practitioner who is dedicated to improving the lives and opportunities of individuals experiencing various challenges including financial hardship, poor mental and physical health and other complex issues. Youll bring knowledge and experience of supporting adults with physical and mental health issues, and understand the impact of ill health, deprivation and discrimination on adults, children, young people and communities. Wandsworth is a highly diverse community and we would welcome applicants with additional language skills, who would be able to deliver support in the service users first language.

Job responsibilities

The role is multifaceted and includes delivering emotional and practical short term support to guests and service users predominantly accessing the WellFamily Service, and receiving up to 6 sessions of emotional and practical support. The majority of referrals are made by GPs but we also take self-referrals and referrals from other partner agencies. The role will also involve flexibility to work with foodbank guests, who receive two sessions of support under a similar model. The constraints of working one day a week means that the role will be predominantly office based, whilst offering online support to our guests and engaging with all the other facets of the role.

The role involves providing holistic assessments, using a solution focused model to provide emotional and practical support, and signposting to improve health and well-being outcomes for individuals and their families. The longer term outcome of providing early help support is to reduce an escalation into specialist and statutory services (including a reduction in the number of GP appointments made by the guest).

JOB DESCRIPTION

Key tasks and responsibilities:

1. To undertake holistic assessments and collaborative Health and Well-being plans with individual service users/guests, identifying support needs to ensure maximum engagement in improving mental health and well-being. Providing up to 6 sessions of WellFamily or Foodbank support.

2. To provide friendly, understanding, safe and non-judgemental advice and support to our guests, and allow them to explain their problems and empower them to set their own priorities. Undertake holistic assessments and formulate a collaborative plan, identifying support needs and referring internally or to other specialist agencies as appropriate.

  1. To offer follow up sessions by appointment, signposting and making referrals to statutory and voluntary agencies, according to the individual needs of the guests, service users and their families.

  1. To establish and maintain effective liaison with stakeholders including foodbanks, Primary Care Networks and GP Surgeries, Citizens Advice Wandsworth, health, voluntary and children and adult social services. Facilitating regular meetings with referrers and engaging with relevant partnership and community events.

  1. To ensure information on sources of voluntary and community support is up to date at all times to enable effective and accurate signposting and linking of individuals with services. To work in partnership with all voluntary and community organisations to use the multiagency database to make referrals.

  1. Have a comprehensive understanding of the value and experience of using evaluation systems and outcome tools to measure progress and encourage co-production.

  1. Provide quarterly comprehensive outcome focused reports and case studies detailing the progress of the service against its key performance indicators.

  1. To keep records of your work to the required safeguarding standards, adhere to GDPR, confidentiality, information sharing protocols, and provide monitoring information as required.

  • 1. Educated to level three or above with a recognised professional qualification in social work, counselling, health, education or equivalent, and evidence of a commitment to continuing learning and professional development. Qualifications and experience as a counsellor, and registration of a professional body is preferable.
