Job summary

The Hastings Primary Care Network is in search of a compassionate and committed Frailty Specialist Care Coordinator to join our rapidly expanding team. In this role, the Frailty Care Coordinator will play a crucial administrative role, collaborating with practices and the broader community care team to ensure that frail patients receive appropriate interventions aimed at assisting them in maintaining as much independence as possible.

This entails coordinating various clinician and social care services, as well as assisting individual patients in identifying and accessing support services provided by the voluntary sector or healthcare providers.

Main duties of the job

  • Reviewing Paramedic home visits to try to proactively identify frailty and intervene before crisis
  • Arrange multidisciplinary teams between Primary & Secondary Care, Hospice and Care Home staff etc to collaborate and optimise patient care.
  • Liaise with the frailty nurses based in A&E to identify issues and barriers, being the main point of contact.
  • Support discharges from hospital, ultimately creating robust processes and pathways between Primary, Secondary and Community Care.
  • Improve data quality coding of frail and housebound patients in the GP medical system.
  • Visit each of the eight PCN practices to review their individual processes.
  • Work closely with the Clinical Leads to ensure smooth working and training between the Care Homes and themselves.
  • Facilitate referrals to health and care services, being the main point of contact.
  • Help improve patient access to GPs.
  • Ensure practices are aware of how to keep up to date with the most useful information for all services in the Summary Care Record

About us

Hastings and St. Leonards Primary Care Network is a large PCN comprising eight GP practices that serve a population of 100,000 patients. The network team works closely with each of the constituent practices to provide collaborative services, resolve shared problems and explore shared opportunities. The PCN team includes a dedicated infrastructure of Clinical Directors, Practice Representatives, Operational and Financial Management and Clinical and Non-Clinical Service teams including Health Coaching, Clinical Pharmacy, First Contact Physiotherapy, Social Prescribing and Occupational Therapy.

Experience

Essential

  • Strong interpersonal and communication skills to engage effectively with patients, families, and healthcare teams
  • Excellent organisational skills with the ability to manage multiple tasks and prioritise effectively
  • Proficiency in using electronic medical record systems and other relevant software (training available)
  • Compassionate and empathetic approach towards patients and their families
  • Ability to work independently and as part of a collaborative team
  • Proactive work style, must be able to solve problems autonomously
  • Must be able to take responsibility for the remit of tasks and progress, unsupported or proactively seek support from the available resources whenever necessary
  • Excellent interpersonal skills and a willingness to work as part of a team
  • Minimum GCSE (or equivalent) Maths and English grade C
  • Confident and effective communication skills, both verbally and in writing
  • The ability to work under direction, meeting deadlines as required
  • Show a capacity to work alone and the ability to work under time pressure
  • Manage and prioritise multiple tasks

  • Previous experience of working in General Practice
