Job summary
We are seeking an enthusiastic clinical pharmacist to deliver medicines
optimisation services within our
Primary Care Network (PCN) and to support development and delivery of a new
pharmacist led spirometry service.
The successful candidate will work as a part of a multi-disciplinary
team in a patient facing role to promote high-quality prescribing and optimise
the management of long-term conditions across the PCN. The role will include
providing spirometry service along with the comprehensive clinical services to
complement testing.
Main duties of the job
Responsible to: Clinical Director, Associate Clinical Director PCN Pharmacist Lead, Practice Manager
Accountable to: Hastings and St Leonards PCN
Salary: £ 42,840- £57,830 a year depending on previous experience and postgraduate qualifications.
Job Purpose
The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in all aspects of medicines optimisation across the PCN. This will include structured medication reviews and diagnoses and management of asthma and chronic obstructive pulmonary disease, with direct patient contact,and may include contributing to:
- Diagnosis and management of other long-term conditions
- Minor illness
- Management of medicines on transfer of care
- Reviewing systems for safer prescribing
- Contributing to repeat prescription authorisations and reauthorisation.
- Action acute prescription requests
- Addressing both the public health and social care needs of patients
- Contributing to achievement of The Quality and Outcomes Framework (QOF), The Investment and Impact Fund (IIF) indicators and locally commissioned quality improvement schemes
- Undertaking clinical audit
- Developing, promoting and implementing the high quality, evidence based and cost-effective use of medicines in primary care in a way that maximises the benefits of medicines to patients and service users and which minimises risks (clinical, legal and financial).
About us
Hastings and St. Leonards Primary Care Network is a large PCN comprising 9 GP practices that serve a population of 100,000 patients. The network team works closely with each of the constituent practices to provide collaborative services, resolve shared problems and explore shared opportunities. The PCN team includes a dedicated infrastructure of Clinical Directors, Practice Representatives, Operational and Financial Management and Clinical and Non-Clinical Service teams including Covid Vaccination, Health Coaching, Clinical Pharmacy, First Contact Physiotherapy, Social Prescribing and Occupational Therapy.
