Job summary

This is an exciting new post, split between The University of Birmingham and Harborne Quinton PCN, Our Health Partnership.

You will be an experienced Pharmacist who will act within their professional boundaries supporting and working alongside a team of Pharmacists in a GP Superpartnership.

Working as part of a multi-disciplinary team in a patient facing role, you will work within a 2 practice Primary Care Network, as a PCN Clinical Pharmacist undertaking structured medication reviews to proactively manage patients with complex polypharmacy.

Additionally, you will lead on developing a placement programme across Birmingham and Solihull ICB practices for pharmacy undergraduate students at the University of Birmingham.

This new post is full time over 5 days and involves 0.6 FTE (22.5 hours) a week working with the University of Birmingham and 0.4 FTE (15 hours) working in the Primary Care Network as a Clinical Pharmacist.

Main duties of the job

The post holder is an experienced clinical pharmacist who has successfully completed the CPPE training pathway for Primary Care and is a qualified Independent Prescriber.

When at the University of Birmingham, the role will include:

Teaching and advising undergraduate and postgraduate students within a variety of clinical and pharmacy subject areas in an established teaching programme.

Leading, managing and supporting on aspects of the primary care placement programme, supporting the delivery of placements across Birmingham and Solihull, within the School of Pharmacy.

When working as a PCN Clinical Pharmacist, the role will include:

Working within the wider multi-disciplinary team to deliver the PCN DES contract (including delivering Structured Medication Reviews), utilising independent prescribing qualification and skills to improve patient care.

Supporting general practice staff with regards to prescription and medication queries. Performing medicines reconciliation on transfer of care and implementing systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the PCN.

Our Health Partnership was set up by local GPs who are passionate about providing high quality primary care and using their time and skills effectively to benefit patients.

We are currently a GP partnership of 33 practices with 42 surgeries. 176 GP partners and circa 80 salaried GPs in Our Health Partnership serving around 300,000 patients in Birmingham, Sutton Coldfield, Wolverhampton, Solihull and Shropshire.

The partnership offers a shared administrative and management structure, cutting down the time doctors have to spend on admin. It opens up economies of scale to get best value from budgets. It has the resources to develop innovative services and effective partnerships with local hospitals and care services. And it can access new funding streams that are only available to large GP organisations.
