Job summary
We are seeking a Lead Care Coordinator to join our growing team at Our Health Partnership, working for and across South East Shropshire Primary Care Network (SES PCN).
The successful candidate will be part of a dynamic and forward-thinking PCN led Multi-Disciplinary Team (MDT). Leadership will be provided by the PCN Clinical Director and Consultant Pharmacist at OHP. We are seeking applicants looking to work between 28 and 37.5 hours per week. Flexible working arrangements, including home working are possible although attending the PCN practices will be necessary.
Main duties of the job
We are looking for individuals who will coordinate the work of healthcare professionals and non-clinical staff including volunteers involved in the care of patients registered at GP practices within the wider PCN population. The role includes responsibility to work with the Network Services Manager and the practices to support and ensure compliance with the reporting requirements of the PCN DES (direct enhanced services) including the Investment and Impact fund (IIF).
A key part of the role of a care coordinator is to lead performance of the Care Homes MDT (multi disciplinary team) across the PCN. The successful candidate will act as a point of contact for professionals who visit and work in care homes, such as MDT members and in-reach specialists. They will support the pratices MDT to deliver care home rounds. Collation of information on people requiring an MDT review in addition to providing coordination, secretarial and administrative support to the MDTs and the community teams within the PCN.

Our Health Partnership was set up by local GPs who are passionate about providing high quality primary care and using their time and skills effectively to benefit patients.
We’re currently a GP partnership of 34 practices with 43 surgeries. 176 GP partners and circa 80 salaried GPs in Our Health Partnership serving around 300,000 patients in Birmingham, Sutton Coldfield, Wolverhampton, Solihull and Shropshire.

SES PCN is a Primary Care Network made up of 9 GP practices offering care to a population of 58,000 patients. The networks spans from Ironbridge and Albrighton in the north and East of the area to Cleobury Mortimer in the South of the county. The area is accessible for all the major towns in Shropshire, The Black Country and Birmingham.

Overall responsibility for ensuring
the weekly PCN led MDT meetings occur (including the weekly virtual Care
Home(s) MDT) and the smooth running of integrated care within the team setting.
The key role of the lead Care coordinator will be to monitor and report for the
PCN on the care home DES.

Manage reporting required and
associated within the DES specifications for required services.

The
Care Coordinator will work closely with GPs and other Primary Care
professionals within the PCN to identify a caseload of identified patients,
ensuring that the appropriate support is made available to them and their
families.

Tackling
health inequalities with a focus on local priorities is pivotal as well as
providing much needed advice and guidance to a diverse population of patients.

To
work with the Network Services Manager and Clinical Leads within the PCN to
facilitate delivery and reporting of the PCN DES.

Signpost
team members, service users and carers to relevant services
·
To ensure
the IT requirements for recording activity are adhered to in collaboration with
other team members
·
To provide
agreed performance/activity data within the MDT and PCN and wider OHP
organisation.

Demonstrates ability to work as a member of a
team.

Work with service
users, PCN practices and partners e.g. Care Homes to ensure new referrals are
logged and allocated
