Job summary

This rewarding Band 6 Hospice Community Nurse role offers the opportunity to deliver specialist, compassionate care to children and young people with life limiting or life threatening conditions. You will provide holistic, responsive nursing support across the community, including in family homes, hospitals and wider care settings, ensuring each child receives the right care at the right time. This role requires experience of delivering high quality clinical care and the ability to work collaboratively with Community Childrens Nursing Teams, hospital specialists, palliative care services and hospice colleagues to ensure seamless, joined up support for families. You will be the first point of contact for many families following referral, taking time to understand their needs, explore their understanding of hospice care and guide them towards the most appropriate support. You will provide symptom management, anticipatory guidance and responsive care during periods of uncertainty, and you may act as a hospice key worker for some children, coordinating services to maintain continuity. You will also participate in a 24 hour on call rota to support children approaching end of life at home, offering hands on nursing care, practical advice and emotional support. Working closely with our psychosocial team, you will help deliver truly holistic care for every family.

Main duties of the job

The Community Nurse plays a key role in delivering high-quality, holistic palliative care to babies, children and young people with life-limiting conditions, and their families. As a core member of the Community Team, the post holder provides skilled nursing care in the family home, wider community settings, and across Shooting Star Childrens Hospices services.

They act as the first point of contact for families following acceptance to the service, carrying out comprehensive assessments and ensuring that care is responsive, individualised and family-centred.

Working both autonomously and collaboratively, the Community Nurse contributes to seamless multi-disciplinary care by liaising closely with in-house teams, psychosocial services and specialist palliative and symptom care teams.

The role requires excellent clinical judgement, communication and education skills, as well as the ability to support families through complex, sensitive and emotionally challenging situations. As part of a 24/7 service, the post holder participates in the shared community on call rota to meet the needs of planned and emergency care. Shooting Star Childrens Hospices provides specialist care and support to families who have a baby, child or young person with a life-limiting condition, or who have been bereaved. Rated Outstanding by the Care Quality Commission, we support families across Surrey, north-west London and south-west London from diagnosis to end of life and throughout bereavement with a range of nursing, practical, emotional and medical care.

Our specialist care and support for families is available 24 hours a day, 365 days a year. It includes specialist nursing in the community, symptom management and pain relief, overnight respite stays, end-of-life care, specialist bereavement care and a comprehensive range of therapies, groups and clinics for the whole family.

At the heart of what we do are our dedicated staff; their exceptional commitment and professionalism means every family has the opportunity to make every moment count.

It costs £13 million a year to run Shooting Star Childrens Hospices. Just 30% of our funding comes from the government, so we rely on our supporters generosity to keep the service running. We employ 175 members of staff, including 98 nursing and medical staff, and support around 700 families.
