Care Coordinator

Job summary

The Care Coordinator will support both the clinical and administrative functions of the organisation to enhance patient care and service delivery. They will work closely with a range of primary care professionals to manage identified patient caseloads, supporting patients and carers before and after clinical consultations, and ensuring evolving needs are addressed. In addition, they will coordinate the effective running of clinics, including managing SystmOne schedules, advertising shifts, supporting service mobilisation, and maintaining accurate data for governance and reporting. The coordinator will also facilitate MDT coordination, manage patient pathways by arranging investigations and follow-up appointments, and ensure timely treatment in line with national standards.

Main duties of the job

Proactively identify and work with a cohort of people to support their personalised care requirements, using the available decision support aids.

Help people to manage their needs, answering their queries and supporting them to make appointments.

Ensure that people have good quality information to help them make choices about their care.

Provide coordination and navigation for people and their carers across health and care services, alongside working closely with social prescribing link workers, health and wellbeing coaches and other primary care roles.

General Administration and note taking as appropriate.

Manage the adding and editing of clinics on SystmOne.

Respond to and directing telephone calls, including taking messages when required.

Monitor the Federation email accounts.

Support the Federation management team in the collection and submission of relevant data.

Arrange appointments for investigations and/or follow up appointments for additional patient tests as required by the clinician.

Assist the effective management of the patient pathway ensuring that all necessary appointments, including the booking of outpatient clinics and waiting lists, are arranged on time and that patients receive timely treatment in accordance with national standards.

North West Leicestershire GP Limited is a federation of 12 General Practices who work collaboratively to offer services at scale for our patients. The Federation is responsible for the running of the North West Leicestershire Primary Care Network which offers services to approximately 116, 000 patients.

  • Excellent communication skills, listening, written and verbal.
  • Excellent interpersonal skills a skilled and sensitive communicator, confident in dealing with staff, people and patients.
  • Excellent working knowledge of and application of Microsoft Office packages.
  • Ability to work both autonomously and as part of a multidisciplinary team.
  • Ability to deal with complex facts/situations requiring analysis, interpretation and comparison of a range of options.
  • Experience of coordinating and liaising with multiple stakeholders or individuals to meet specified outcomes.
  • Experience of analysing and interpreting data and producing performance reports for stakeholders.
