Job summary
Social Prescribers support other health and care
professionals with an option to refer patients to a range of local, non-clinical services when their health can no longer be improved through traditional medical interventions.
Our Social Prescribers support
individuals to take greater control of their own health in a way that makes
sense to them and enables them to feel connected and supported to facilitate
sustainable change.
Social prescribing schemes can involve a variety of
activities which are typically provided by voluntary and community sector
organisations in locations that feel comfortable for the individual. Activities that support
feelings of contribution and connection with others.
We recognise that community-centred ways of working can be more effective than more traditional services in
improving the health and wellbeing of individuals and have a good track-record of providing this.
