Transition Nurse (Registered Nurse)

Job summary

We have an exciting opportunity for a Registered Nurse to support the transition of young people with Life Limiting or Life-Threatening Conditions who access hospice care and are transitioning from Childrens to Adult Social Care, Education and/or Health Services.

This role will require you to travel across all Andy's catchment areas so a clean driving license and access to a vehicle is essential. Flexibility is also required to be able to undertake visits at a time convenient to the young person and their family.

Main duties of the job

You will lead on Transition between childrens and adult services for the organisation, acting as a resource to colleagues regarding transition issues and offer specialist advice as required. This may include offering training and support to other professionals in supporting young people with complex needs.

To be proactive in helping young people to plan for their future, ensuring plans can be adapted in response to changing wishes and circumstances.

Promote an innovative and progressive attitude to the continual improvement of transition for young people through research and evidence-based practice.

  • Experience of managing a caseload of young people to meet targeted outcomes.
