Job summary
We recognise the importance of Clinical Pharmacists in
Primary Care and the key role that they have is supporting the delivery of the
Network Contract DES and Quality Outcomes Framework (QOF). Delivering a range
of health interventions at Practice level, including developing and managing
medicine optimisation services within Primary Care, systems for safer
prescribing, structured medication reviews (for example of patients with
polypharmacy), reviews of patients with long-term conditions that fall within
their competency, withdrawing medicines no longer needed through NHS led
programmes. For Enhanced Health in Care Homes residents will benefit from
regular clinical-pharmacy-led medicines reviews.
The Clinical
Pharmacist plays a crucial role in our general practices and is of upmost
importance to our local patient population. It serves as a cornerstone in our
pursuit of enhancing health outcomes.
Main duties of the job
The post holder is a pharmacist, who acts within their
professional boundaries, supporting and working alongside a team of pharmacists
in general practice. In this role they will be supported by a senior clinical
pharmacist who will develop, manage, and mentor them and by a GP who will
provide clinical supervision as well as a line manager.
To provide clinical leadership on medicines optimisation and
quality improvement as well as managing aspects of medicines safety and
antimicrobial stewardship.
To work as part of a multidisciplinary team in a patient
facing role and take responsibility for areas of chronic disease management
within the practice and undertake clinical medication reviews to proactively
manage patients with complex polypharmacy.
