Job summary
We have an exciting opportunity to join the Cranbrook Primary
Care Network and shape the health coach role for the delivery of outstanding
primary care in the network area. To take a lead role in design and delivery of
clinical excellence. We are looking for an innovative, motivated health coach
who is looking to join our PCN.
The role will include being responsible for delivering group
and one on one sessions on healthy eating, physical activity and health and
wellbeing to meet with personalised care plans; engaging with patients to
support them in taking an active role in their health; providing advice and
support for both patients and colleagues. In addition, the post-holder will
also support the multi-disciplinary team, in line with the strategic objectives
of the PCN. The role will be supported by the Clinical Director of the PCN.
The right candidate will be educated to GCSE or equivalent
and have undergone training in motivational coaching, behavioral change and
goal setting. The candidate may also hold and NVQ level 3 or other relevant
first level professional qualification or be working towards this. They should
have excellent communication skills, an empathetic style and good
organisational skills with the ability to work autonomously. Experience of
working in primary care would be desirable but not essential to the role. This

Main duties of the job

in partnership with the social prescribing service to connect patients to
community-based activities which support them to take increased control of
their health and wellbeing;
Work
with patients with lower activation scores to understand their level of
knowledge, skills and confidence (their Activation level), when engaging with
their health and well-being and subsequently supporting them in shared
decision-making conversations;

Care Networks (PCNs) in Redbridge are working together as part of the
Redbridge-wide GP federation Healthbridge Direct.

and across the PCNs. We are now at the very start of an exciting journey of
building a team of over 100 additional clinical roles to support and enable the
delivery of services within each of our 4 PCNs. We are building teams within
and across the PCNs to enable peer support and team working both within and
across professions.

interventions such as self-management education and peer support;
Supporting
patients to establish and attain goals that are important to the patient;
Supporting
personal choice and positive risk taking while ensuring that patients understand
the accountability of their own actions and decisions, thus encouraging the
proactive prevention of further illnesses;
Working
in partnership with the social prescribing service to connect patients to
community-based activities which support them to take increased control of
their health and wellbeing;
Increasing
patient motivation to self-manage and adopt healthy behaviours;
Work
with patients with lower activation scores to understand their level of
knowledge, skills and confidence (their Activation level), when engaging with
their health and well-being and subsequently supporting them in shared
decision-making conversations;
Utilise
health coaching skills to support people with lower levels of activation to
develop the knowledge, skills, and confidence to manage their health and
wellbeing, whilst increasing their ability to access and utilise community
support offers; and
Explore
and support patient access to a personal health budget, where appropriate, for
their care and support.
Develop
collaborative relationships and work in partnership with health, social care,
and community and voluntary sector providers and multidisciplinary teams to
holistically support patients wider health and wellbeing, public health, and
contributing to the reduction of health inequalities;
Provide
education and specialist expertise to PCN staff, supporting them to improve
their skills and understanding of personalised care, behavioural approaches and
ensuring consistency in the follow up of peoples goals with MDT input; and
Raise
awareness within the PCN of shared-decision making and decision support tools.
