DGS Health was established by a group of local GP's committed to delivering a range of services designed to provide safe, high-quality care. As part of our initiative, DGS Health will be hosting clinical sessions on behalf of local GP Practices, with a specific focus on Long-Term Conditions patients.

Main duties of the job
The post holder will be required to work across practices in the Dartford, Gravesham and Swanley area. The role will consist of:
- Management of patients with a potential or confirmed Long Term Condition e.g., COPD, Diabetes, Hypertension, Epilepsy, CHD in the clinic environment.
- Development of individual care management plans as appropriate with the patient and other health professionals.
- Refer patients directly to other services/ agencies as appropriate.
- Initiate, perform and contribute to the interpretation of diagnostic procedures as appropriate for chronic disease management
- Undertaking health promotion and NHS health checks

The Dartford, Gravesham and Swanley area is located in North Kent, only a 15 minute commute by train from London St Pancras, and our main GP Federation office is metres away from Ebbsfleet International Rail Station. The area is undergoing considerable housing, retail and leisure development including the Ebbsfleet Garden City Area. The Post holder will work in accordance with the NMC Code of Conduct and DGS Healths values to provide clinical care including routine reviews and health promotion to patients with long term conditions e.g Diabetes/ COPD/Asthma/Hypertension.
Main duties of the job
The postholder will work to agreed protocols and guidelines to provide nursing treatments to patients both independently and as part of a multi-Disciplinary team with nurses, colleagues, and General Practitioners.
The post holder will promote integrated practice between DGS Health and practice teams for shared delivery of care to designated patients of the practice. Further, to ensure clinical effectiveness by monitoring of key quality markers relating to guidelines.
Key responsibilities of the role
- Management of Chronic Disease: Management of patients with a potential or confirmed Long Term Condition e.g., COPD, Diabetes, Hypertension, Epilepsy, CHD in the clinic environment.
- Development of individual care management plans as appropriate with the patient and other health professionals.
- Sharing responsibility with the GPs and Head of Operations to meet the required QOF targets.
- Refer patients directly to other services/ agencies as appropriate.
- Initiate, perform and contribute to the interpretation of diagnostic procedures as appropriate for chronic disease management
- Work with patients to support compliance with, and adherence to, prescribed treatments
- Undertaking health promotion and NHS health checks
- Venepuncture according to local guidelines
- Record clear and contemporaneous consultation notes using Emis
- Read code appropriately and collect data for audit purposes as agreed.
- Checking compliance with and adherence to appropriate treatments using a holistic patient-centred approach.
- Recognising abnormalities.
- Identifying the impact of treatment and implementing or recommending changes as appropriate.
