Job summary

This is an exciting opportunity for a motivated individual to develop skills and experience working within an expanding Primary Care Network (PCN).

The Digital & Transformation Lead will be an integral member of the team which is responsible for delivering projects and services of varying size and complexity within the Primary Care Network. The post holder will be expected to take a lead role with our Access to Primary Care Programme.

The core objective of the PCN Digital and Transformation Lead role is to support the PCN and its practices to deliver ongoing improvement to services. The role will require experience in quality/service improvement with expertise in transformational change and a good understanding of general practice systems and processes, including familiarity with general practice digital systems and how they can be used to deliver improvement.

Main duties of the job

The post holder will need to be able to work well under pressure. The role requires a well organised and flexible approach, with the ability to work as part of a growing, efficient team, and demonstrate the capacity to work autonomously and prioritise and manage a busy workload.

The post holder must possess a pro-active and self-motivated working attitude with the ability to forward plan and react quickly and professionally at all times.

The role will require the ability to work flexibly to meet the needs of the organisation and this may include working outside of core General Practice hours from time to time.

The post will be a hybrid role including working from home. The post holder will also be required to hold a full driving licence and be prepared to travel between locations to support the remit of the role.

work closely with staff working at practices, such as practice managers and staff at the PCN to understand current service models, identify challenges and work with the PCN Leadership to agree priorities for improvement and then support these to be delivered.

develop relationships with peers across other PCNs to share learning and expertise at place or system level.

develop relationships with the ICS and work with the PCN Leadership to discuss and agree local transformation plans and activity, and to support alignment with broader strategy, including the ICSs digital strategy.

work with relevant colleagues in the ICS, and develop relationships with local delivery partners (such as IT service providers and other NHS organisations) to support effective delivery of transformation.

work with the ICS to request specialist IT and estates expertise required to deliver transformation plans, in line with the ICSs delegated responsibilities for GPIT and GP estates. The post holder will also work with the ICS to bring in other specialist support as required e.g. in analytics, evaluation, organisational development etc.

work with the PCN leadership to help develop strategy related to transformation and improvement and helping align this with larger scale strategy and initiatives, particularly at ICS level; they will contribute to collaborative problem solving with other system partners, patients and communities; focussing their time on the planning, implementation and evaluation of practical improvements that make a difference to patients and staff in the PCN.

  • Good understanding of the Digital agenda within the NHS and Social Care/Care Sector
  • Good understanding of current healthcare policy relating to primary care and integrated care
  • Awareness of positive disruption that digital change can bring
  • Good understanding of the Data Protection & Security Toolkit and Information Governance
  • Good understanding of confidentiality and data protection, including GDPR
  • Experience in Digital/IM&T project management or transformation experience in a Health or Social Care environment
  • Experience in project delivery through the entire project lifecycle from initiation to delivery
  • Awareness of how population health management can support the reduction of health inequalities
