Job summary

We have an exciting opportunity for a Healthcare Support Worker to join our highly experienced care team. We are looking for dedicated healthcare professionals with a passion for providing holistic family centred care to a caseload of very special and inspiring children, young people, and their families. As a Tŷ Hafan Healthcare Support Worker you will be supported in main and develop your skills to a high standard, in addition to working closely with Nurses and the wider team of professionals to provide truly intuitive and creative care that goes beyond your usual tasks. Main duties of the job

As a Healthcare Support Worker your responsibility will be to contribute ine the delivering safe and supportive care to children resident in the hospice, working as part of a multi-disciplinary team to ensure continuity of care.As a tŷhafan Healthcare Support Worker tasks will be delegated by a Registered Nurse, in consideration of your level of skill and confidence. You will be responsible for providing care in line with our individualised care plans, designed by Registered Nurses in compliance with current guidelines, regulations and policies.Our skilled team provide specialist care for children and young people with a wide range of complex conditions therefore, at Tŷ Hafan your work will be extremely varied, and deeply fulfilling.

To deliver person-centred care for babies, children and young people with predictable care needs and assessedas being stable and allocated by the registered nurse in charge.

To participate fully in a range of delegated duties including admission and discharge to and from the hospice,and care requiring the use of technology or specific techniques in accordance with care plans.

To follow care plans, observe any changes in the baby, child, or young persons condition, and promptly seekingadvice and reporting back to the nurse in charge.

To communicate effectively with children and families.

To maintain accurate, clear and contemporaneous records and documentation in line with Tŷ Hafans recordkeeping procedures.

To participate in planning and delivering play and support activities, including hydrotherapy.

To provide support and assistance to other team members when they are carrying out nursing procedures,therapy treatments, or play.

To safely and effectively use assistive technology and mobility equipment.

To actively participate in care planning and review meetings.

To actively participate in governance and health and safety processes, including risk assessments, highlightingany concerns and reporting all incidents, accidents and near misses promptly.

To participate in the management of clinical stock levels and report any requirements.

When required, to participate as a team member in other activities which are part of providing care and support,including laundry, meals, collecting equipment, ad hoc light domestic duties, and outings.
